Horizon PACE is excited to announce an amazing opportunity to be part of Horizon PACE’s program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare team at Horizon PACE will approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state. Horizon is opening 3 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. Under the supervision of the Home Care Coordinator, the Personal Care Assistant is responsible for providing and assisting participants of Horizon PACE with activities of daily living, restorative and supportive care. Assists with activities within the home to maintain the home environment. Assists in monitoring and reporting the health status and providing a safe environment for the participant. Functions as a member of Interdisciplinary Team.
Position Responsibilities:
· Consults each day with the Home Care Coordinator (or designee) to be updated on Horizon PACE participant’s status, changes in plan of care and to confirm scheduled visits.
· Under the direction and supervision of the Home Care Registered Nurse assists participants with the following duties:
· Completes appropriate documentation of services provided following Horizon PACE
program policy
· Follows all Horizon PACE program policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines
· Maintains the confidentiality of all company procedures, results, and information about participants, clients or families
· Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions
· Acts within the scope of his or her authority to practice
· Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications
· Participates in and support Quality Improvement Initiatives
· Follows all Horizon PACE program policies and procedures and applicable Occupational Safety and Health Administration (OSHA) safety guidelines
· Performs other duties as required or requested
