32 Hour FT MSW Position

We are currently hiring for:

Full Time 32 Hours per week, Master of Social Work

4- 8hr Shifts: Sunday, Monday, Wednesday & Thursday, 8:30 am -5:00 pm

Responsibilities:

• Completes psychosocial assessment and develops social work plan of care
• Conducts follow-up visits based on goals and plan of care. Involves patient and family in education and counseling on death/dying issues, resource identification and coordinating support.
• Assess patient/family social and emotional factors in their ability to cope with the terminal illness and death.
• Communicates with members of the interdisciplinary team.
• Attends Interdisciplinary Meeting to assist in the development, review, and implementation of patient/family care plan.

Requirements:

• Master’s degree from a school of social work accredited by the Council on Social Work Education
• One (1) year of social work experience in a health care setting

Preferred Qualifications:

• Palliative and/or hospice care experience
• Experience in delivering homecare or field case management

Pay: From $28.00 per hour
