Hours: Full time (37.5 hours per week)

Position Summary:

The Emergency Services Care Coordinator will serve within Horizon's emergency services team to support the Department of Behavioral Health and Disability Services' (DBHDS) requirements for collaborative discharges for clients admitted to State hospital facilities. In addition, this position will provide similar discharge coordination services to high utilizers of emergency rooms and private hospital admissions.

Required Education and Experience:

• Bachelor's degree in psychology, social work, counseling or human services field
• One (1) year of relevant clinical experience
• QMHP-A

Knowledge, Skills, and Abilities:

Knowledge of severe mental illness, trauma informed care, and substance use disorders; Strong communication skills in order to effectively communicate with internal and external stakeholders; Ability to establish strong therapeutic relationships with clients and their support network; Ability to collect, analyze and summarize relevant clinical information in order to present and advocate for clients during clinical treatment team meetings; Ability to manage multiple cases simultaneously along with the ability to handle sudden changes in a client's status or clinical discharge plan; Ability to communicate effectively verbally and in writing.
