Hope Youth is Hiring a Case Planner- PINS Diversion Near Bellmore, NY

Hope For Youth has an opening as a Case Planner for the Nassau County PINS Diversion program. This position reports to the Program Supervisor. Hope For Youth is a nonprofit organization striving to provide positive outcomes for children, adolescents, and families. The Case Planner will provide a service plan to youth and their families to improve the overall functioning and wellbeing of the household. Additionally, counseling domestic To be a successful Case Planner you must be willing and able to do the following always:

  • Function within Agency's policies and procedures as outlined in the Employee Manual.
  • Function within the agency policy and procedures as outlined in the program guide and applicable codes of conduct.
  • Provide support to the clients and families-i.e., the knowledge, skills and tools necessary in order for them to maintain the child in their home.
  • Achieve successful outcomes within the guidelines outlined in NYS, County and HFY policy and procedure (shared).

How you will spend your time in this job:

  • Maintaining regular contact with the youth and their families at least once per week and implementing the various parts of the program model (including evenings, nights and weekends) for caseload (approximately 15-20) throughout Nassau County utilizing your own vehicle.
  • Providing case management services, life coaching, parent skill training, and make all collateral contacts as necessary.
  • Regularly reviewing the service plan with the youth and family.
  • Completing the initial CANS-NY and all subsequent CANS-NY, as well as attending school meetings and court appearances with the families as necessary and be responsible for discharge/transition planning.
  • Documentation and related paperwork
  • Supervision, staff meetings, treatment team meetings and conferences
  • Training and staff development
  • Other related assignments

Qualifications:

  • BSW (or Bachelor's in a related filed) plus 2 years related casework experience or MSW
  • Excellent written, verbal and computer skills
  • Flexible schedule to accommodate clients and program
  • Bilingual Spanish/English speaking preferred
  • Access to a reliable vehicle that can be used on a regular basis for home visits
  • Physical Requirements: read/write reports and log entries, walking/standing for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, driving own vehicle
