Hope For Youth has an opening as a Case Planner for the Nassau County PINS Diversion program. This position reports to the Program Supervisor. Hope For Youth is a nonprofit organization striving to provide positive outcomes for children, adolescents, and families. The Case Planner will provide a service plan to youth and their families to improve the overall functioning and wellbeing of the household.
