Job Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. Your role will involve crisis intervention, counseling, and connecting clients with necessary resources. Duties:
- Conduct comprehensive assessments of clients' needs and develop individualized care plans
- Provide crisis intervention and support to clients in emergency situations
- Coordinate and collaborate with other service providers to ensure clients receive appropriate services
- Assist clients in accessing community resources such as housing assistance (Section 8), childcare, and workforce development programs
- Monitor client progress and make necessary adjustments to care plans as needed
- Provide addiction counseling and support to clients struggling with substance abuse
- Conduct regular home visits to assess living conditions and ensure the safety of clients
- Document all client interactions, progress, and outcomes in a timely manner
Qualifications:
- Bachelor's degree in social work or related field (SUD Targeted Case Manager Preferred)
- Previous experience in case management or related field
- Knowledge of child welfare systems and regulations
- Strong crisis management skills
- Ability to effectively communicate with diverse populations
- Proficient in motivational interviewing techniques
- Familiarity with community resources and services
- Excellent organizational and time management skills
