Hope and Help Center of Central Florida is Hiring a Health Care Advisor for Orange County Jail Near Winter Park, FL

SUMMARY
This position is based out of the Orange County Jail and is responsible for providing HIV and STI/STD testing, prevention education, behavioral interventions, and linkage to care to persons living with HIV and/or STI/STDs and persons at risk of acquiring or transmitting HIV and/or STI/STD.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
? This position is responsible for providing prevention education to those at high risk for HIV or STI?s, as well as case management and linkage to care of those already diagnosed with those infections.
? This will include a detailed assessment of individuals? risk factors for exposure and ongoing monitoring of the efficacy of high-risk behavior modification recommendations.
? This position is also responsible for providing linkage services to individuals newly diagnosed with STI?s and or HIV as well as those with chronic infection in need of care.
? The role decreases barriers to medical and support services, increases consumers? awareness of treatment options, builds/strengthens relationships between the patient, physician, and patient advocate, and fosters patient self-sufficiency through specific advocacy and services.
The individual will be responsible.
OTHER RESPONSIBILITIES
Performs other work-related duties and special projects as assigned.

BACKGROUND CHECK and DRUG TEST
Must be able to pass a Level III Background check.
Must be able to pass a comprehensive test.
EDUCATION and/or EXPERIENCE
Associate's Degree from an accredited college or university and one (1) year of HIV or health education experience or a high school diploma or equivalent and two (2) years of HIV or health education experience.
CERTIFICATES, LICENSES, and/or REGISTRATIONS
Valid Florida Driver?s License required.
