SUMMARY
The EHE Enhanced Case Manager provides case management services to individuals and families living with HIV/AIDS, assists participants with rental assistance to prevent evictions, and provides referrals for supportive services such as medical, dental, and financial assistance to community partners, all intending to maximize self-sufficiency, life skills, and housing stability. The EHE Enhanced Case Manager supports building life skills to attain short and long-term housing goals.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Provides housing case management services for assigned clients, including assessment, planning, linking, monitoring, advocacy, and procedures to achieve successful outcomes.
Provides housing case management to clients with housing insecurities or eligible housing emergencies.
Conducts comprehensive client assessments to collect functional, environmental, psychosocial, financial, employment, housing, educational, and health information as appropriate and develop appropriate case plans.
Develops support systems to meet client needs by identifying and coordinating various available services necessary to maintain independent living self-sufficiency and family stabilization when possible.
Conducts housing crisis intervention for clients with housing emergencies as HUD guidelines require.
Monitors and verifies services provided to each client monthly, determining the quality and effectiveness of services offered.
Assess, orient, and facilitate the entry of clients to the EHE (Ending the HIV Epidemic) program.
Sustains and manages a caseload consistent with the program's annual contractual obligation by applicable quality criteria.
Completes preliminary assessment process for persons seeking services from the agency to collect demographic information and pertinent medical history.
Initiates review of prospective client eligibility and continues review for all active clients.
Completes and makes referrals for assessments and evaluation processes based on client needs and treatment objectives.
Respond to information requests from other specified partner agencies.
Visits agencies, organizations, and individuals to promote housing programs as required.
Monitors client participation in planned services. Initiates revisions to the plan of care and closes or transfers cases.
Obtains authorization for payments from the supervisor and submits all requests for processing according to established time frames.
Attends all regularly scheduled staff and case management meetings.
Participates in agency-wide events that may involve weekends.
Performs other work-related duties and special projects as assigned.
Performance Improvement: Initiates efforts to improve job, program, or agency performance and quality of services. As appropriate, participates in agency performance improvement initiatives or activities (e.g., work teams, committees, etc.).
Initiative/Acceptance of Responsibility: Demonstrates self-motivation and self-reliance. Performs tasks willingly and carries out assigned duties. Performs additional responsibilities as needed/required.

Attendance: Adheres to assigned work hours, use and scheduling of leave, and punctuality.
Training and Education: Maintains compliance with required training by established time frames. Seeks opportunities to increase knowledge and competencies that benefit the position, program, or agency.

QUALIFICATIONS

KNOWLEDGE AND SKILL OF:
Florida Department of Health 500 and 501 HIV/AIDS certification.
Modern office practices, procedures, and equipment.
HIPAA laws and confidentiality requirements.
Understanding of basic Hud housing guidelines.
Excel, Word, and basic computer operations.
HIV transmission, prevention, and disease process.
HIV/AIDS prevention strategies.
Substance Abuse, HIV/AIDS, TB, and Hepatitis as they relate to prevention and medical adherence among special populations.

EDUCATION and EXPERIENCE
Bachelor's degree from an accredited college or university or an associate degree and one (1) year of HIV or health education experience or a high school diploma or equivalent and two (2) years of HIV or health education experience.
CERTIFICATES, LICENSES, and REGISTRATIONS
Florida Department of Health 500 and 501 HIV/AIDS certification
Florida driver?s license
