Your Role

As the Prospective Outreach Manager, you will be responsible for improving Star Ratings and the quality program for ACO REACH by removing patients' barriers to achieving high quality health outcomes. You will work with members of the Quality team to identify and remove barriers to accessing care. You will lead a team of health professionals in closing Star Rating and ACO REACH quality gaps. You will manage the performance of vendors that ensure patients get the care they need. Finally, you will be responsible for coordinating how Honest's direct patient care field teams are closing quality gaps in care.

Primary Functions of the Manager, Prospective Outreach Include:

  • Manage patient outreach by establishing and overseeing execution of developed workplans.
  • Establish, implement, and manage day-to-day relationships with Honest's direct patient care teams in markets.
  • Lead execution of tactics related to removing barriers to patients accessing care to close gaps.
  • Manage and ensure the success of prospective quality vendor solutions.
  • Evaluate and refine prospective offerings, make needed changes based on feedback and outcomes of our Quality program.
  • Lead and manage a team of nurses tasked with removing barriers to accessing care by outreaching patients directly.
  • Perform other related duties as assigned.

How You Qualify

  • 5 experience working with health care providers, payers, and/or quality vendors
  • 3 years experience working on Star Ratings or ACO REACH quality programs
  • 3 years of people management experience with demonstrated skill developing talent
  • Strong project management skills
  • Exceptional communication skills demonstrated through multiple means, including written, oral, presentation slides
