Role: Home Care Social Worker

Responsibilities:

· Assessment

o Conduct comprehensive assessments of client’s social, emotional, financial, and environmental needs.

o Identify strengths, challenges, and goals of clients and their families.

· Care Planning

o Develop individualized care plans in collaboration with clients, families, and healthcare professionals.

o Ensure care plans address the unique needs and preferences of clients.

· Coordination of Services

o Arrange and coordinate home-based services such as medical care, counseling, and community resources.

o Facilitate communication and collaboration among healthcare providers and support networks.

· Advocacy

o Advocate for client’s rights and access to appropriate services, benefits, and resources.

o Assist clients in navigating complex healthcare and social service systems.

· On-call responsibilities.

· Any other duty requested to maintain the operations of the business, including caregiving duties.

· Record Keeping / Journaling.

· Performs miscellaneous job-related duties as assigned.

Requirements:

The appointed Scheduler for the agency must meet the following qualifications:

· Strong interpersonal and communication skills and the ability to work effectively with various constituencies in a diverse community.

· The ability to gather data, compile information, implement needed information, and prepare reports to share with the team, including family and clients.

o Practice reflective supervision and engage in peer support activities to process challenging cases and experiences.

Review:

· Regularly review and update care plans in response to clients’ changing needs and goals.

· Seek feedback from clients, families, and colleagues to identify areas for improvement and ensure quality service delivery.

Physical Qualifications:

· Able to work an average of 40 hours per week

· Able to bend, climb, stoop, and stand an average of 5 hours per day

Job Type: Part-time

Pay: $16.00 - $18.00 per hour
