Homespace Corporation is Hiring a Case Coordinator Near Buffalo, NY

The Case Coordinator is responsible for the initial and ongoing assessment of all clients in key areas (health, safety and well-being); assisting clients in developing realistic goals; making referrals for services; maintaining positive relationships with community service providers; ensuring permanency and regular family contact; working proactively with the team to ensure productivity; and monitoring the progress of clients, and for the “hands-on” work that is done to improve and show progression with each client’s Independent Living (IL) skills. Utilization of Trauma Informed Care focus is essential in this position.

Essential Position Responsibilities:

  • Use the strengths-perspective and client-focused approach to develop a service plan with each client; work collaboratively with team to implement service plan; document updates and changes to plans regularly using internal methods and NYS Connections system.
  • Assess and plan family goals toward reunification, permanency, or other appropriate goals. Promote confidence and competence in all clients by setting realistic goals and positively acknowledging progress towards those goals.
  • Ensure accurate and timely documentation of group attendance, group summaries, goal planning, Family Assessment and Service Plans (FASP), Service Plan Reviews (SPR), health and medical information and Permanency Hearing Reports (PHR). etc. in the NYS Connections system, and appropriate billing systems (Accumedic), document updates and changes.
  • Ensure successful transition to services after aging out, Make referrals to Y-PREP, Belmont, SNAP, etc.
  • Incorporate biological family in service planning and delivery to promote permanency and encourage healthy relationships, as appropriate and deemed necessary by Local County Department of Social Services (LCDSS) and Local County Family Court (LCFC).
  • Comply with federal, state, and local legal requirements/regulations related to client population within all programs.
  • Protect and maintain client’s rights by keeping location and personal information confidential. Comply with all applicable HIPAA requirements.
  • Cooperate to resolve conflicts utilizing verbal de-escalation techniques from Therapeutic Crisis Intervention to ensure safety of the clients and their children.
  • Provide clinical services that maintain each client’s dignity and trust within the professional relationship while maintaining professional boundaries.
  • Ensure all appropriate staff members has access to Case Planner Updates, safety plans, and assessment tools in order to ensure that safety needs are met for all clients.
  • Contribute to agency effectiveness by working cooperatively with other team members by regular supervision, clinical meetings, Service Plan Reviews, joint staff meetings and all other collaborative meetings to meet the needs of the clients, team and agency.
  • Ensure that all medical needs are provided to clients and frequency standards for appointments are met.
  • Utilize Clinical On-Call in times of crisis.
  • Communicate with team members and supervisor about any concerns related to safety and imminent risk.
  • Advocate for client’s strengths and needs with other service providers.
  • Provide experiential learning opportunities in the community with every client in order to build on and track IL goals.
  • Provide means for clients to visit with family and supportive adults in order to enhance relationships and build positive support system.
  • Discuss strengths with clients related to interpersonal skills (i.e.: communication, emotional management, coping skills, healthy relationships, etc.) independent living skills, and parenting)
  • Use knowledge of Child Welfare System, the effects of trauma, and the strengths-based perspective to guide work with all clients and children in the program.
  • Maintain dignity and respect of all clients.
  • Maintain accurate and up-to-date case records to ensure successful Utilization Reviews.
  • Provide advocacy for existing gaps in services and when needed services are non-existent.
  • Maintain communication with Clinical Supervisor and Director of Services regarding intakes, discharges, and pertinent census information.
  • Complete Quarterly and Annual Census Report with the Clinical Supervisor and the Director of Services
  • Identify and make referrals to appropriate community resources to address needs and achieve goals.
  • Attend staff meetings and in-service trainings.
  • Promptly report any problems, unusual behaviors, observed changes in client behavior, or medical conditions in compliance with documentation procedures.
  • Assist with the development of new community linkages and renew existing linkages.
  • Ensure safety and cleanliness of living spaces by completing weekly home inspections and identifying and reporting any concerns to resident to incorporate concerns into skill building.
  • Complete Transition Plans with residents aged 17.5 and update every 6 months, as necessary. Transition plans must be documented & uploaded in Connections and shared with LCDSS.
  • Utilize Casey Life Skills Assessment and other assessments to provide in-depth analysis of client’s needs as it related to independent living.
  • Work with the client to develop an understanding of the importance of housing, how it is related to maintaining a budget, and the consequences of poor decisions as well as the safety nets available in case the client makes those decisions.

Quality Assurance:

  • Comply with federal, state, and local reviews requirements related to client/employee population and operations.
  • Work with state reviews, expectations, codes and follow up.
  • Monitor and ensure appropriate and timely documentation is completed
  • Ensure the supervision, safety, and comprehensive well-being of each and every client/child in the program at all times.
  • Ensure a safe and secure environment 100% of the time.
  • Cooperate to resolve conflict by using appropriate responses to the situation.
  • Maintain certification in CPR/FA through the duration of employment.
  • Participate in all trainings deemed necessary for the position and utilize skill set consistently (ie: TCI)
  • Attend all staff meetings and scheduled trainings.
  • Contribute to agency effectiveness by working cooperatively with other team members.
  • Adhere to all aspects of the Homespace Employee Handbook.
  • Assist in furthering the mission, policies and procedures of Homespace as a whole.
  • Perform all other duties as assigned by the assigned Supervisor/DoYS and /or the Executive Director.

Minimum Qualifications:

  • Bachelor’s Degree in social work, education, psychology, rehabilitation counseling or related field plus work experience in human services, Licensure preferred.
  • Valid New York State driver’s license and an ability to transport.
  • If driving one’s vehicle, must maintain valid inspection and automobile insurance.
  • Must provide proof of insurance upon hire and when requested.
  • Must maintain active NYS Defensive Driving class every 3 years, upon expiration.
  • Ability to carry, lift or host heavier objects at times, including children, must have mobility enough to regularly climb at least one flight of stairs, utilizing multiple levels.
  • Evidence of commitment, sensitivity, and flexibility in working with adolescent parents.
  • State mandated background checks to include the following: Clearances through the NYS Central Register, the NYS Sex Offender Registry, NYS Justice Center Staff Exclusion List, fingerprinting, and criminal history check from IndentoGo, NYS OMIG Medicaid Exclusion List, monthly Medicaid Exclusions database check and the SS Death Master Check.
  • Completion of a work physical upon hire and yearly thereafter.
  • Other qualifications as deemed necessary by the Executive Director.
