The Homeless Prevention Council is a nonprofit providing personalized case management services to support self-sufficiency and stability for residents of Lower Cape Cod. HPC is seeking a Case Manager to work with our dedicated team.

The Case Manager will:
Qualifications
Experience in case management and/or a housing-related field is preferred. Proficient with Microsoft applications and database systems. Precise and confidential record keeping, excellent oral and written communication skills and a commitment to teamwork are essential. Must have a valid MA driver's license and provide own transportation. Candidates who speak languages other than English are encouraged to apply.
