As a full-time Homeland HomeHealth employee, you would be eligible for:
As Homeland HomeHealth’s Weekend RN you would provide skilled nursing care to home health patients as prescribed by the physician and in compliance with the Home Health Care Medicare Conditions of Participation, Pennsylvania Home Health Care licensure laws and agency policies and procedures. Provide the initial and comprehensive nursing assessment of patient needs and collaborate to develop the patient centered, home health plan of care. Implement the plan of care and reevaluate and revise the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions. Work closely with the assigned case manager and other members of the health care delivery team to ensure all patient and family care needs are being met. Provide guidance to weekend Licensed Practical Nurses (LPN) and HomeHealth Aides. Coordinate the weekend schedule, assigning visits to meet the home health patient's needs.
