Homedic Hospice Care is Hiring a Hospice Nurse Near West Covina, CA

About us

Homedic Hospice in Los Angeles County. We have an immediate need for a Registered Nurse for patients in the areas listed below.

We are looking for a Hospice Registered Nurses to provide care and support to terminally ill patients. Your presence and services should bring comfort to the person who faces the end of their lives and help them reach acceptance and peace of mind.

Currently seeking an RN to provide care for homebound patients in Los Angeles County, San Bernardino County, Riverside County, and Orange County.

Salary: Starting Wage $50 per hour (wage depends on type of visit, experience, and distance from patient)

Duties Include:

· Completes and initial, comprehensive, and ongoing recertification assessments of patient and family to determine hospice needs. Provides and complete physical assessment and history of current and previous illness(es).

· Provides professional nursing care by utilizing all elements of nursing process.

· Assesses and evaluates patient’s status by writing and initialing plan of care, regularly re-evaluating patient and family/caregiver needs, participating in revising the plan of care every 2 weeks, if needed.

· Uses health assessment data to determine nursing diagnosis.

· Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.

· Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.

· Counsels the patient and family in meeting nursing and related needs

· Provides health care instructions to the patient as appropriate per assessment and plan.

· Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.

Requirements:

· Registered Nurse with current licensure to practice professional nursing in the state.

· Current licensure in state and CPR certification.

· Hospice experience with post-mortem care preferred.

· Excellent observation, verbal, and written communication skills, problem solving skills, basic math skills: nursing skills per competency checklist.

· Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, keeling or crouching. Visual acuity and hearing to perform required nursing skills.

· Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

· Bilingual highly desired! (Cantonese, Mandarin, Japanese, Tagalog, Spanish, etc.)
