As a Home Care Provider III, you will provide home care to seniors and people with disabilities requiring home care services to continue living independently in the community. This position provides services to newly referred clients, clients transitioning home from an institution, and substituting clients' existing services. When required, this position provides rapid response services, interacts with hospital discharge staff, and implements client Care Plans, providing feedback and input to both the client and Homebridge staff on client progress, issues, barriers, etc.
Home Care Duties
Basic home care tasks to be performed are described in detail in the written Care Plan. Basic duties to be performed fall into five categories as described below:
  • Domestic Services: cleaning, sweeping, meal preparation and cleanup, escorting to doctor’s, assistance with oral hygiene, etc.
  • Personal Care Services: assistance with grooming, washing, toileting, stand-by assistance in and out of shower, assistance with incontinence or self-administered oxygen, etc.
  • Complex Personal Care: assistance with bladder and bowel care (incontinence, emptying bag), transfers, Hoyer lift, dementia care, respiration assistance, etc.
  • Paramedical Care: Injections, administering medications, wound care, bladder care, bowel care, application of ointments, respiratory care, bandaging, etc.
Specialized Care
  • Hospital Discharge Care Assistance: transitions client from institutional care to home
  • Identifies and partners with clients who need heavy cleaning and work with clients who have just had a heaving cleaning
  • Works with clients who have chronic behavioral diagnoses (such as Dementia, Bipolar, Schizophrenia, Hoarders & Collectors etc.) as trained by Homebridge
  • Assists with and reminds client about medication as well as record logs accurately
Skills and Abilities
  • A sincere desire to help people in need and provide them compassionate care
  • Must be dependable and able to communicate and follow directions
  • Dependable and able to work and travel in all areas of San Francisco
  • Ability to demonstrate critical thinking skills and responsible decision making
  • Work with multiple clients and manage day to day challenges. Demonstrate an ability to adapt to different personality types and rapidly changing client behavior
  • Develop trust with client, and work with client’s emotional needs to overcome obstacles to care
  • Tailor professional behavior, presentation, and care delivery to suit individual client’s needs
  • Understand and implement subtle and specific directions in care plan geared towards client’s mental health in addition to their physical care
  • Understand Motivational Care Management care plan model and work within that model
  • Have an in-depth knowledge of Home Care Provider responsibilities, standards, and proper techniques, and proven ability to demonstrate proper techniques
  • Possess strong interpersonal communication skills with ability to communicate effectively/professionally with medical personnel
  • Willing to work with the lower-income population, chronically ill, active substance addiction, and low self-compliance with health/medical care plan
  • Self-organized, can prioritize competing assignments, and multi-task
Additional Requirements
  • Must be available to work full time, including Saturday and/or Sunday and Agency holidays
  • Must have already graduated from the STEPs to Success HCP II training
  • Must have basic comprehension of written and verbal English
  • Must use Promo phone
