Home Watch Caregivers of Bridgewater is Hiring a Director of Nursing for a Home Care Agency Near Bridgewater, NJ

Homewatch of Bridgewater NJ is seeking a Director of Nursing to join our team.The Director of Nursing is responsible for maintaining our accreditation with the Commission on Accreditation for Home Care (CAHC).The Director of Nursing is a key member of our Client Service Team along with the Scheduler and Owner.You will be responsible for opening cases, completing initial assessments, developing a plan of care, and training Caregivers.You will be required to communicate any changes to the client’s condition in real time to the Client Service Team.

The Director of Nursing will provide support to our Caregiver team and establish policy and procedures. The Director of Nursing is also the designated contact for all COVID-19 related questions, testing requirements, and potential exposure next steps.

The Director of Nursing will oversee a team of Registered Nurses.It is expected that the Director of Nursing establish standing team meetings and provide support, coaching, and feedback in real time.

The Director of Nursing is expected to lead by example and embrace and promote teamwork and communication throughout the organization.

Essential duties and responsibilities include the following:

Administrative:

·Regulatory & Commission on Accreditation for Home Care (CAHC) compliance

oPolicy manual updates & logs

oClient Visit Frequency

oCG Competency Testing

oOngoing training compliance

oQuarterly PQI reviews

·Caregiver Medical files

oInitial medical compliance

oAnnual PPD compliance

·Caregiver/Client file audits

·Commission on Accreditation for Home Care (CAHC) Required paperwork

oBasic journal entries

oDischarge Summaries

·Caregiver Performance Review

oQuality of Care Input

·Play an active role in weekly client service roster review meeting

·Caregiver Training Program

oOrientation

oAnnual Meetings (TBD)

Client Care Support:

·Complete initial assessment

·Complete a home visit every 60 days

·Develop Plan of Care

·Complete Caregiver Case Orientation

·Complete Caregiver skill assessments

·Family Follow up as necessary

·Medication follow ups

·Post Hospital Visit

·Six Month Re-assessment

·Client ‘triage’ support

Caregiver Support:

··COVID-19/Flu Shot/PPD Effort

Team Management:

·Manage a team of Nursing Supervisor

·Provide real time feedback, coaching, and support

·Monitor overall Nursing Supervisor/client activity to ensure monthly targets are being achieved

·Facilitate regular team meetings

Marketing:

·Facility Support

·Community Support as requested

On-call Support:

·Assist with being on-call one Weekend a Month 

·24/7 ‘On call’ availability, as needed

Key Performance Metrics:

·Ensure each client is seen at least one a month (self and team)

·Develop team and meet with them on a regular basis

·Pass all Accreditation Audits

Education & Experience Requirements:

Minimum: Must be a licensed Registered Nurse with a bachelor’s degree in a health related field.Minimum of 3 years of clinical experience in nursing with at least two years in home health care. Knowledge of or ability to learn all applicable Federal, State, local and Joint Commission regulations. Quality Improvement experience required.

  • Must have flexibility with working hours as occasional before or after-hours care services may be required on short notice
  • Ability to communicate clearly and effectively
  • Must be tactful, professional, and empathetic at all times
