Job Summary: Provides in-home and center-based case management and supportive services. The Senior Family Support Partner is an experienced home visitor who brings specialized training/expertise applicable to the position and demonstrates leadership ability. Senior Family Support Partner also assists in the creation and implementation of training plans and audits.
Duties and Responsibilities:
Conducts client assessments consistent with programrequirements.
Provides case management services including identification of clients' needs and goals on a case plan with ongoing monitoring and follow-up on progress towards thosegoals.
Teaches and demonstrates goal setting and decisionmaking.
Provides support to families in identifying and obtaining communityresources.
Maintains a flexible working schedule to accommodate families'schedule.
Maintains accurate and timely case records documenting all servicesprovided.
Maintainsmonthlyreportinganddataentryasrequiredbythecontract,includingachievementof productivitygoals.
Monitors participants' progress in developing skills needed to assure positiveoutcomes
Creates training plans and training tools for new and existing staff.Pa
Assists in the training and development of new and existingstaff.
Conducts monthly file audits of client files and programdatabase.
Assures security of confidential information andmaterials.
Networks and develops relationships with various community serviceagencies.
Attends agency, program and collaborative meetings asassigned.
Accepts direction and reports to team leader for directsupervision.
Attends and participates in training asappropriate.
Other duties asassigned.
Qualifications:
Education: A minimum of a Bachelor's degree or equivalent in social work, psychology, education and/or related human services field. Specialized training/expertise in area relevant to position.
Demonstrated experience in in-home, low income, family-based services and/or commensurate experience may qualify. Knowledge: Knowledge of community resources. Abilities: Demonstrated ability to be culturally sensitive and respect diversity. Ability to provide support, structure, empathy, sound judgement, insight into human behavior and family relations. Skills: Effective communication and leadership skills required. Must be a team player and be able to work independently. Computer literate as appropriate. Bilingual in Spanish, Asian, or Pacific language preferred. Other Requirements: Must have a valid California driver's license, insurance and a reliable vehicle and be willing to use it on thejob.
Physical Demands:
Sitting for long periods of time in the homes of client families, in the office or at required meetings. Daily traveling in car to clients or to agency and community meetings.
Frequent note taking and recording in case files by hand.

Office space isshared
The noise level fluctuates from quiet tonoisy.
