Prerequisites:

· Valid Driver's License & show proof of Vehicle Insurance

· 21 years of age or older

· Complete any compliance, license of registration required by the State (We can help guide you through the process)

· Pass background check and drug screening

· Use the WellSky Personal Care App and must have a device that supports the most up to date version of iOS or Android
