• You have a valid RN License or LPN? (Preferred)
• You have at least 1 year of Experience with Client Care Coordinating?
• You have at least 1 year of Experience in Home Care, Home Health, Healthcare?
• You have excellent attention to detail, computer skills, and people skills?
• You have a valid D.L. & vehicle with insurance?

Your Responsibilities!

• Care consultations with potential clients and family members, client/CAREGiver introductions and quality assurance visits with existing clients.
• Use our personal approach to determine each individual client’s needs to provide solutions and create individualized service plans.
• Continuously evaluate the care plans through ongoing communication and home visits to ensure high quality care, client satisfaction and retention.
• Oversee all functions performed by the CAREGivers.
• Stay organized and properly plan your day/week/monthly schedules for home visits.
• Recognize potential issues, but also be able to address them and create plans to ensure health standards are being met.
