HomeHealth Admission & OASIS RN Responsibilities:

·Perform Admission / "Start ofCare" OASIS Visits throughout an area of the Metro

·Evaluate and assess clients for homehealth services and create their plan of care

·Recommend services from other disciplinesthat have not yet been ordered by the physician

·Collaborate with clients, families,physicians, and other members of the home care team

HomeHealth Admission & OASIS RN Qualifications:

Licensedas a Registered Nurse in the state of Minnesota

·Driving is required; must have a reliablevehicle with current insurance and a valid Driver's License

·Prior Home Health Nursing experience

·Proficiency with Medicare & OASISdocumentation

·Demonstrated ability to work autonomously

·Excellent at planning, organizing, andcommunicating
