OASIS RN Responsibilities:
As a Home Health Admission Nurse, your duties will include:

• Performing Admission / "Start of Care" OASIS Visits throughout the Metro area.
• Evaluating and assessing clients for home health services and creating customized plans of care.
• Recommending additional services from other disciplines as needed and collaborating with the healthcare team.

OASIS RN Qualifications:
To succeed in this role, you'll need:

• A valid Registered Nurse license in the state of Minnesota.
• A reliable vehicle, valid Driver's License, and current insurance.
• Prior experience in Home Health Nursing and proficiency in Medicare & OASIS documentation.
• Strong autonomy, organization, and communication skills.
