Job Duties:

·Meet with clients one-on-one in their own homes

·Create and update clients’ plan of care based on their diagnosis and goals

·Perform semi-skilled nursing tasks-based client plan of care

·Educate client and family regarding specific practices to reach goals

·Coordinate client cares with physicians, case managers, and interdisciplinary team members

·Supervise Home Health Aide with caregiving instruction when necessary

·Daily documentation of client visits using electronic health records system

·Create and manage your weekly work schedule

·Participate in nurse on-call rotation

·2-5 days per week (at your option); office in Plymouth

Qualifications:

·Active MN RN License in good standing

·Valid driver's license with good driving record

·Willingness to travel daily (local)

·Organization, communication, and time management skills

·Be able to utilize and navigate EMR system on a company-issued laptop
