Patient Advocate - Home Health - full-time needed!

Nightingale Home Healthcare was established 26 years ago with one purpose – to put our patients first. Nightingale provides patients with around the clock care and support. Patient Advocate Needed! Nightingale Home Healthcare is expanding rapidly, and we are seeking a Patient Advocate to promote our home health services to physicians, patients, hospitals.

Duties Include:

· Promotes Nightingale Home Healthcare to physicians, patients, hospitals and the community

· Works collaboratively with directors to focus on targeted market

· Provides plan of action to directors on a monthly basis

· Utilizes excellent communication skills

· Maintains professional image with all staff and customers

· Prioritizes areas of focus

· Represents Nightingale Home Healthcare at various functions

· Maintains knowledge of home healthcare rules and regulations

· Maintains contact with physicians and other referral sources to solve any issues that occur between Nightingale Home Healthcare and the physician and/or other referral source. Refers them to Administrator if unable to assist them

· Maintains current knowledge of new trends in the home care industry

· Performs other duties as assigned

Qualifications


Qualifications:

· Bachelor’s degree in business related field, marketing degree preferred

· 2 years of patient navigator home health services experience

· Valid driver’s license, clear driving record, and car registration

· Ability to attend 2-day orientation at our Stamford office

· Ability to work company operational hours of 8 AM – 5 PM
