Job Description

- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the supervisor
- RN (Home Visit), RN is responsible to transporting completed client charts to the client’s home within 7 days of
chart preparation to obtain the required signatured for the state of North Carolina’s mandatory documents for
the below reasons:- Initial Admission to the home care agency – Home Care Perfections
- 90-day Supervisory visits
- Annual Assessments
- Care Plan Updates as needed
- RN (Agency Forms), RN is responsible for signing all required documents in the Employee charts upon hire, annual training and evaluations.
- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Effective Communication
- Time Management
- Valid driver’s license
- First aid/CPR certification required
- Basic Comprehensive Computer Skills (Microsoft, Excel)
