Responsibilities

• Participate in the planning and coordination of patient care in conjunction with the DON and the physician's plan of care and orders
• Complete Medicare SOC and Oasis E documentation
• Follow physicians orders and ensure compliance with plan of care
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change
• Prepare clinical/progress notes and all required documentation
• Perform all duties and responsibilities always adhering to Florida chapter 464 - Nurse Practice Act

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing
• Current state license as an RN in good standing
• Current CPR certification
• Valid driver’s license and reliable transportation
• At least ONE year experience as an RN at a Medicare Home Health Agency
• Medicare Documentation and Oasis E experience required
