Job Purpose or Summary:

The Home Care Liaison works individually and within their team to generate qualified prospects for services that result in meeting goals for adding new clients. Responsible for managing referral marketing and community outreach programs. Establishes and maintains relationships with referral sources. Responsible for meeting work production and financial goals of the sales team. Base salary is $45,000 plus commission.

Essential Duties and responsibilities:

· Building and maintaining relationships with referral sources in the community to generate brand awareness and new clients for Home Care Assistance. Ensuring the satisfaction of the referral sources you work with to generate ongoing, long-term business.

· Building priority lists, advancing relationships, tracking your activity effectively and completing all the other necessary steps of successful referral marketing as outlined by our Business Development Process

· Representing Home Care Assistance in networking groups, at events, on committees and in other community settings

· Responding to prospective client inquiries and ensuring they receive the information they need to move forward.

· Conducting assessments with prospective clients as needed, addressing their questions and concerns, building relationships with their families; and converting them into clients.

· Communicating effectively within the team to ensure that our care plans meet the clients’ needs, that stakeholders and referral sources are satisfied, and that relevant information is communicated in an effective and timely manner.

· Providing after-hours support for referral partners, prospective clients, and active clients

· Serving as a back-up to the care management team to participate in client care, assessments, home visits, and call rotation as needed.

· Maintaining a pulse on the strategic position of Home Care Assistance within the market, identifying new opportunities to enhance our reputation or build our referral SAMPLE 2Rev 09/2022 base, helping to implement new programs and initiatives, and helping us to identify talented new additions to our local team as required.

· Building the strength of our brand in the local market Knowledge and Skill Requirements:

· Bachelor’s degree in business or related field; or similar experience

· Previous experience in health care management or marketing, preferably in home care to elders

· Demonstrated knowledge of industry

· Demonstrated ability to work independently.

· Excellent verbal, written communication skills

· Demonstrated organizational skills, flexibility, assertiveness, and team orientation.

· Ability to represent business at community events, conferences, and conventions. May require some travel.

· Requires ability to set up and tear down Displays and event booths. This job requires lifting, carrying, and loading items up to 50 lbs.
