Works under the supervision of the Clinical Manager. Assesses, plans, implements, evaluates, and supervises individual care to patients in both the ambulatory setting. Serves as link between patients, providers, and other services needed for transition of care. Ensures appropriate education and support focused on teaching self-care behaviors. Accepts the responsibility for directing nursing care during transition period and ensures appropriate hand-off to care management as needed. Collaborates with acute and ambulatory nursing staff, care managers and social work. Acts as liaison to Cleveland Clinic specialists for coordination of care. Assists in patient education activities including genetics, chemotherapy administration, general infusion, oral medication and radiation therapy.

Education, Work Experience and Licensure:

· RN certification required with experience in Hematology/Oncology preferred.

· Holds an active nursing license recognized by the Ohio Board of Nursing.

· Computer skills and ability to learn clinical information/ data warehouse reporting systems.

· Presentation, negotiation, problem solving and conflict resolution skills.

· Excellent verbal and written communication skills.

· Three to five years of progressively responsible experience is preferred, primarily in the areas of Hematology/Oncology or case management.

· Prior healthcare leadership is desirable.

· Evidence of working independently and effectively with other professionals and patients to achieve optimal member health.

· Active OCN certification or willingness to become certified within 2 years.
