SUMMARY OF JOB:

Coordinates care of a group of residents through phone calls and/or visits to apartments in conjunction with the Office Coordinators under the supervision of Nursing Supervisor. Involves coordinating transportation, HCA services, appointments and various needs of the group. Provides personal care as needed to clients.

RECRUITMENT REQUIREMENTS:

• High School diploma or equivalent preferred
• 2.5-3 years’ experience in a Home Care role involving direct contact with the individual receiving services.
• Computer experience required
• Knowledge of medical terminology
• Well organized, accurate and attentive to detail
• Excellent communication and follow up skills
• Certified HHA/CNA or qualified for employment under PA state regulations
• Must be able to make meaningful observations and report changes in patient condition
• Must be able to demonstrate appropriate skill level based on Competency testing
• Must have valid and current driver’s license, registration and insurance for own vehicle

LICENSE AND REGULATORY REQUIREMENTS:

• Certified HHA/CNA or qualified for employment under PA state regulations
