A Brief Overview
Quality Coordinator is responsible for a variety of quality and educational tasks for Homecare. Identifies issues and works to resolve clinical and utilization issues affecting the service line. Integrates the quality metrics and performance improvement programs within the clinical and financial aspects of the service line. Participates in the planning and coordination of quality to include patient/family satisfaction surveys, audits for compliance, and policies, programs and plan reviews. Participates in the analysis of data regarding trends in utilization, outcomes and quality. Participates in the educational activities within the service line
What you will do

  • In collaboration with the Director of Home Care & Quality Team, determines the strategic approach to process change to meet regulatory requirements
  • In collaboration with Quality Team members, ensure compliance among the nursing, rehab and other support staff across the service line
  • Participates in the development of education to support the service line through the development and implementation of quality improvement activities and maintains the activities as appropriate
  • Collaborates among the departments within the service line to continually seek improvement in processes that affect the quality and delivery of care and assists in the implementation of practices that improve service
  • Establishes mechanisms to identify issues and track the corrective action
  • Participates in the service line’s performance improvement (PI) program which is comprehensive and integrated and compliant with all regulatory bodies, licensing authorities and agency policies
  • Participates in the development, implementation and evaluation of best practices in Home Care
  • Develops and maintains effective communication with all staff to facilitate problem identification and resolution
  • Works closely with the leadership team within the service line to resolve issues related to patient care
  • Participates in quality improvement (QI) activities such as but not limited to focused chart audits, clinical chart audit, recertification process and Transfer/Discharge OASIS
  • Review and reports results quarterly to the agency and administration of the medical center
  • Serves as liaison for home health aide staff by reviewing care plans, visit compliance, oversight of annual competencies and resolving issues related to home health aide services
  • Manages general orientation to department, management of HR files and oversight of CPR/Annual assessments
  • Perform joint visits across the service line to ensure infection control, patient safety and quality care
  • Manages CMS and insurance requests and appeals
  • Performs other duties as assigned


Education Qualifications

  • Bachelor's Degree In Nursing Required and
  • Master's Degree Preferred


Experience Qualifications

  • 1-3 years Minimum of three (3) years quality improvement activities in a healthcare setting Required or
  • Clinical nursing experience in homecare. Required and
  • Management/operations experience in homecare Preferred


Knowledge, Skills, and Abilities

  • Knowledge of federal and state standards applicable to homecare required.
  • Knowledge of governmental and other regulatory standards, requirements, and guidelines related to quality improvement.
  • Strong working knowledge of ongoing monitoring of care delivery in homecare and quality management principles, tools and techniques


Licenses and Certifications

  • RN - Registered Nurse - State Licensure and/or Compact State Licensure Current New Jersey Registered Nurse License Upon Hire Required
