Holy Mary Home Health Care is Hiring a Director of Nursing - Home Health Agency Near Palatine, IL

Job Summary:

We are seeking a dedicated and experienced Director of Nursing to oversee our Home Health Agency. The ideal candidate will provide leadership and support to our nursing staff while ensuring the delivery of high-quality patient care in accordance with the Agency’s objectives and policies, through planning, coordination, and implementation.

Responsibilities:

  • Develop and implement care plans in collaboration with physicians and healthcare team
  • Oversee nursing administration, including staffing, training, and performance evaluations
  • Ensure compliance with all regulatory requirements, including Medicare guidelines
  • Maintain accurate medical records and documentation
  • Coordinate with other departments to provide comprehensive patient care
  • Utilize knowledge of physiology to assess patient conditions and recommend appropriate interventions
  • Manage nursing operations in various settings such as assisted living, home health, nursing homes, and acute care facilities
  • Coordinate, and schedule, the daily workload of the agency, utilizing Agency personnel to their fullest capacity.
  • Provide daily supervision of district nurses, and Home Health Aides, to ensure the quality of services and maintenance of standards.
  • Review, and investigate, the clinical content, diagnoses, medical history, medications, fee status, and other procedures, from the records of patients.
  • Direct staff toward the appropriate use of community resources.
  • Provide the teaching and counseling necessary to enhance the delivery of services, and the professional development, of nursing and Health Aide staff.
  • Identify, and evaluate, patient needs and appropriateness of home care, by reviewing the initial intake screening form before assignment of direct care staff.
  • Participate in community related functions, as necessary.
  • Assist with the orientation of new professional staff, to familiarize them with Agency policies and procedures.
  • Ensure that all patient documentation and files are ready for coding and billing
  • See patients in the home setting, as needed.
  • Participate in Joint Commission audits, interdisciplinary team conferences and represent Agency, in the community, as requested.
  • Ensure that the agency is Joint Commission survey ready and in compliance with Joint Commission standards and policies.
  • Perform Quality Assurance (QA) for all patient charts
  • Review each nurse's schedule for recertification, discharges, and Aide supervisory visits.
  • Review weekly schedule submitted by nurses for changes or updates.
  • Perform other duties as directed, by the Administrator.

Requirements:
- Active Registered Nurse (RN) license in Illinois
- Proven experience in Home Health nursing leadership role
- Strong understanding of Medicare regulations and nursing best practices
- Excellent communication and interpersonal skills
- Ability to multitask, prioritize, and delegate effectively
- Proficiency in medical records management systems

Preferred Skills:
- Experience in developing and implementing care plans

- Prior knowledge with use of Axxess software
- Knowledge of home health and assisted living environments
- Familiarity with nursing administration practices
- Understanding of acute care settings and nursing home operations

Job Type: Full-time

Pay: $95,000.00 - $105,000.00 per year
