Reports to: Registered Nurse/Licensed Vocational Nurse

General Description: Provides direct patient care according to each resident’s established plan of care under the direction and supervision of a licensed nurse.

Duties and Responsibilities: include the following. Other duties may be assigned.

  • Responsible for providing direct care to residents which may include assistance with or total performance of bathing, dressing, toileting, personal hygiene, incontinence care, feeding, hydration, and transfers.
  • Responsible for knowing the individualized plan of care for each assigned resident as noted on the ADL documentation form. Reviews form daily for changes prior to beginning care and documents care delivered. Document’s care refused or not delivered according to facility guidelines and informs licensed nurse of the situation.
  • Is alert to changes in a resident’s condition and reports symptoms and/or behaviors to licensed nurse in a timely manner. Promptly reports to licensed nurse any resident who is not eating/drinking, has a change in urinary or bowel status, a change in mental status, or change in skin condition so the resident can be assessed and treated in a timely manner.
  • Encourages resident independence and decision-making in the care process to the extent possible. Does not automatically perform a task for the resident that the resident is capable of doing for himself/herself and assists only as needed. Recognizes when a resident is capable, but unwilling, to assist in own care and encourages but does not force the resident’s self-care assists whenever needed in any circumstance. Notifies licensed nurse of any problems with care delivery.
  • Responsible for recording all food and fluid intake and ensuring that each resident consumes as much as possible at each meal. Offers nutritional supplements if resident consume less than 50% of a meal and records how much of the supplement was consumed. Notifies the licensed nurse if problems are incurred during a meal or resident refuses food or fluids. Weigh’s residents according to the plan of care and reports current weight to licensed nurse as well as documenting weight on the ADL record.
  • Responsible for obtaining monthly vital signs on each assigned resident according to plan of care. Documents vital signs on ADL records and reports any abnormalities to the licensed nurse in a timely manner.
  • Maintains a high level of awareness for potential threats to resident/employee safety and takes prompt action to report or resolve the problem. Maintains resident rooms in clean and tidy condition and takes immediate action to remove any items on the floor or in the walkway areas of the room to prevent falls. Notifies licensed nurse of any persistent, unresolved safety issues.
  • Responsible for insuring, to the best of his/her ability, the safety of each assigned resident using fall prevention programs as noted on the resident’s plan of care. Does not leave residents unattended in bathroom or shower if the resident has a diagnosis of dementia, cannot/will not follow safety precautions, or needs assistance with transfers. Transfer’s residents using mechanical lifts as much as possible, avoiding manual lifting to ensure safety of both resident and personnel. Uses caution during transfers to avoid falls or injuries related to the transfer. If the resident care plan specifies a particular method of transfer to be used with that resident, MUST use this mode of transfer every time.
  • Responsible for notifying licensed nurse immediately of any resident incidents, such as falls, skin tears, bruises, burns, etc. so the resident can be assessed and treated in a timely manner.
  • Compliant with all Holly Hall administrative, personnel, and nursing policies and procedures.
  • Adheres to infection control guidelines for body fluid exposure and prevention of disease transmission. Consistently removes gloves and washes hands between resident contacts. Bags all soiled linens in the resident’s room and ties linen bags securely in the room prior to bringing to the soiled linen/trash area. Empties trash as needed to prevent odors, ties bags securely in the room prior to bringing to the soiled linen/trash area.
  • Responsible for keeping clean linen carts tidy and covered and does not leave items on top of carts. Uses carts for soiled linen and trash pickup and promptly takes full carts to soiled linen/trash room and empties. Does not leave soiled linen or trash in halls or resident doorways. Places carts in the cart storage areas when not in use to prevent residents from removing or soiling clean linens.
  • Compliant with all CNA certification standards as governed by the State of Texas. Responsible for obtaining necessary continuing education to meet the regulatory standards of the Texas Department of Aging and Disability Services for long term care employment.
  • Responds to residents, family members, physicians, nurse practitioners, and others in a professional and courteous manner. Works cooperatively with other nursing personnel and other departments to resolve problems and minimize disruption. Works as a team with other assigned CNAs to accomplish resident care tasks efficiently and safely. Notifies licensed nurse of any resident or family complaints, problems or concerns in a timely manner to facilitate prompt resolution.

Qualifications


Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and abilities required. Education and/or Experience: Certified Nursing Assistant certification from a college or technical school; or one to two years related experience and/or training in long term care; or equivalent combination of education and experience.

Language Skills: Ability to read and interpret documents such as safety rules, nursing documents, and procedure manuals; ability to write in a legible and comprehensible manner; and ability to speak effectively with residents, family members, physicians, other departments, and employees of Holly Hall.

Mathematical Skills: Must be able to add, subtract, multiply, and divide using whole numbers, decimals, and percentages.

Reasoning Ability: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form; and ability to deal with problems involving a limited number of variables in a standardized situation.

Certificates, Licenses, Registrations: Current certification as a Certified Nursing Assistant in the State of Texas.

  • Must be able to speak, read and understand verbal and written instructions in English; and must be able to be understood clearly and easily by residents as English is the prevailing language at Holly Hall Retirement Community.
  • Must be able to make independent decisions when circumstances warrant such action.
  • Must be able to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the public.
  • Must be able to work harmoniously with other personnel.
  • Works beyond normal working hours, on weekends, and in other positions temporarily, when necessary.
  • Sits, stands, bends, lifts, and moves intermittently during working hours.
  • Is subject to frequent interruptions.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  • Must function independently and have flexibility, personal integrity, and the ability to work effectively with other staff.
  • Must be in good general heath and demonstrate emotional stability.
  • Must be able to lift, push, pull, move, and manipulate at least 50 pounds.
  • Must be able to assist in the evacuation of residents.
