Holisticare Hospice is seeking a dynamic and dedicated individual to join our team as a Patient Care Advocate. Territory will be Ocean and Monmouth Counties
As a Patient Care Advocate, you will support and partner with current Liaisons to identify new referral sources and maintain ongoing relationships to increase program census and preference for hospice services. Patient Care Advocates use their hospice knowledge, empathetic communication skills, and marketing abilities to initiate one-on-one dialog with referral sources, families, and prospective patients to help explain and educate on hospice services. Our Patient Care Advocates are hospice experts and serve as a resource to our Liaisons and referral sources to ensure extraordinary patient and customer service.
