HOLISTIC HOME AND HOSPICE is Hiring a Director of Clinical Services/Nurse Practitioner (Full-time) Near Milwaukee, WI

The Director of Clinical Services NP is responsible for the overall direction of hospice clinical services. Maintains the quality of patient care in accordance with the Agency’s objectives and policies, through planning, coordination, implementation and evaluation of the hospice services in accordance with the patient's plan of care.

A staff person serving in another position within the hospice may also be the coordinator of clinical services if he/she meets the requirements for both positions and can adequately carry out the duties of both positions.

Responsibilities

  • Examine patients and their medical records
  • Order and study diagnostic tests (blood tests, X-rays etc.)
  • Diagnose health conditions and illnesses
  • Propose treatments for chronic and infectious diseases
  • Prescribe medications
  • Maintain accurate records and schedules
  • Inform patients about their health conditions and prescribed medications
  • Educate patients and their family members on diseases, epidemics and prevention
  • Oversee patients’ overall health
  • Promote good health practices
  • Advance professional knowledge and conduct research
  • Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.
  • Provide supervision and guidance to clinical staff to ensure the quality of services and maintenance of standards.
  • Perform chart audits.
  • Gather data for QAPI and participates in QAPI activities.
  • Appraise clinical staff performance during their probationary period and annually, provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.
  • Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff.\
  • Oversees the maintenance of patient clinical records and assures compliance with local, state and federal laws as well as ACHC standards of care.
  • Assist with the orientation of new staff.
  • See patients in the home setting as needed.
  • Participate in audits, IDG meetings and QAPI.
  • Maintains adequate and appropriate Medical Supplies.
  • Responsible for orientation of new personnel as well as in-services for current staff.
  • Responsible for ensuring Medicare, State and ACHC regulations are followed by all clinical staff.
  • Assume responsibility for self-development by continually striving to improve his/her Nursing/Management practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.
  • Proven experience as Nurse Practitioner in a clinic, or other setting
  • Expertise in primary care, acute care and chronic care practices
  • Knowledge of health and patient care regulations
  • Excellent communication skills
  • Strong ethics
  • Team spirit with a positive attitude
  • Patience and cool-headedness
  • Willingness to continue gaining knowledge and clinical experience
  • Valid nurse practitioner license
  • Master’s degree; PhD is a plus
  • Ability to market and deal tactfully with customers and the community.
  • Knowledge of business management, governmental regulations and ACHC standards.
  • The ability to communicate well, both verbally and in writing.
  • Must have the ability to demonstrate self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement.
  • Primarily a desk job which involves minimal and occasional lifting of medical supplies and charts.
  • On occasion, one may be required to bend, stoop, reach and move patient weight up to 250 pounds lift and/or carry up to 30 pounds.
  • Travel is required to and from patient's home when supervision of care is scheduled.
  • The ability to access patients’ homes which may not be routinely wheelchair accessible is required.
  • Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
  • Coordinates hospice care by working with other team members as well as with the patients/family members to ensure needs are met.
  • Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.
  • Perform evaluations for hospice care along with informational visits as needed.
  • Utilize the nursing process to provide professional nursing care.
  • Consult with the attending physician and Hospice Medical Director concerning changes in patients’ condition requiring order changes and/or changes in the plan of care.
  • Coordinate patient services with other members of the IDG as well as contracted services, when needed.
  • Submit complete, accurate and relevant clinical notes in a timely manner.
  • Submit patient visit schedule weekly.
  • Participate in IDG and case conferences.
  • Discuss with the supervisor if IDG determines that the patient would benefit from contracted services.
  • Take responsibility for ensuring patients’ plans of care are kept current.
  • Performs on-call duties (including holidays) per policy and agency need.
  • Participate in staff development meeting.
  • Continually strive to improve his/her nursing care skills by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations and/or individual research and reading.
  • Provide education to patients/family members regarding the dying process, plan of care and offers guidance with healthcare decisions.
  • Provide dietary counseling, taking into account health issues and dietary changes at the end of life.
  • Perform death visit per agency’s policy.
  • Participate in the patient’s discharge planning process.
  • Maintain an on-going knowledge of current drug therapy.
