We are currently seeking Registered Nurse Case Managers to provide onsite care to our hospice and palliative care patients. The best candidates for our positions are professionals who have a passion for end-of-life care and comfort. The most appropriate candidates for this position enjoy working independently and have excellent communication skills.

Responsibilities:

• Work with Holistic Care Hospice IDT to set treatment goals for patients and implement those goals.
• Assist patients and their families through the dying process and help them understand their options.
• Work with Holistic Care Hospice Care Coordinators to provide excellent care for patients and their families
• Timely reporting and documentation of patient's condition.

Areas include Cerritos, Downey, Anaheim and surrounding areas.

Job Types: Full-time, Part-time

Pay: $55.00 - $65.00 per hour
