The Patient Navigator assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes. The Patient Navigator coordinates research data, community physicians, hospital resources into a seamless model of access and care benefiting patients, participating physicians, and family members. The Patient Navigator must know and perform all clerical/administrative support duties associated within the Department. The Patient Navigator will be responsible for assuring that all information is maintained in accordance with FDA, ACR, MQSA, and JCAHO standards.
Education: High school graduate or equivalent. (Preferences: Graduate of LPN or RN program OR Graduate of radiology or ultrasound program)
Experience: Prefer at least 6 months -1 year recent experience
