A Mental Health PRP (Partial Hospitalization Program) Case Worker plays a pivotal role in facilitating the recovery and rehabilitation of individuals dealing with mental health challenges within a structured program. PRP Case Workers are responsible for assessing clients, developing personalized treatment plans, coordinating services, and offering guidance to empower clients on their journey towards improved mental well-being.

Key Responsibilities:

Assessment and Treatment Planning:
- Conduct comprehensive assessments of clients to identify their mental health needs, strengths, and treatment goals.
- Collaborate with a multidisciplinary treatment team to create individualized treatment plans.

Coordination of Services:
- Help clients access and navigate a range of mental health services, including therapy, medication management, and community resources.
- Advocate for clients within the program and with external service providers to ensure seamless care.

Skill Development and Rehabilitation:
- Support clients in acquiring essential life skills, including coping strategies, communication, and problem-solving abilities.

  - Crisis Intervention and Support:
    - Be prepared to respond to crisis situations by employing de-escalation techniques and providing emotional support.

    - Maintain accurate and confidential client records, including progress notes, treatment plans, and service coordination efforts.

- Qualifications:
  - Bachelor's or Master's degree in psychology, social work, counseling, or a related field.

  - Solid understanding of mental health disorders, evidence-based therapeutic approaches, and trauma-informed care.
  - Excellent communication skills, both written and verbal, for effective client interaction and reporting.
