Job Description:
A Mental Health PRP (Partial Hospitalization Program) Case Worker plays a pivotal role in facilitating the recovery and rehabilitation of individuals dealing with mental health challenges within a structured program. PRP Case Workers are responsible for assessing clients, developing personalized treatment plans, coordinating services, and offering guidance to empower clients on their journey towards improved mental well-being.


Key Responsibilities:

  • Assessment and Treatment Planning:
    • Conduct comprehensive assessments of clients to identify their mental health needs, strengths, and treatment goals.
    • Collaborate with a multidisciplinary treatment team to create individualized treatment plans.
  • Coordination of Services:
    • Help clients access and navigate a range of mental health services, including therapy, medication management, and community resources.
    • Advocate for clients within the program and with external service providers to ensure seamless care.
  • Skill Development and Rehabilitation:
    • Support clients in acquiring essential life skills, including coping strategies, communication, and problem-solving abilities.
        • Provide education on mental health topics, relapse prevention, and self-advocacy.
      • Crisis Intervention and Support:
        • Be prepared to respond to crisis situations by employing de-escalation techniques and providing emotional support.
        • Collaborate with crisis management teams to ensure clients' safety and immediate needs are addressed.
      • Documentation and Reporting:
        • Maintain accurate and confidential client records, including progress notes, treatment plans, and service coordination efforts.
        • Prepare regular reports for treatment teams, supervisors, and regulatory agencies.
      • Advocacy and Community Integration:
        • Advocate for clients' rights and assist them in accessing community resources, employment opportunities, and educational programs.
        • Foster positive community relationships and facilitate clients' participation in external support networks.
    • Qualifications:
      • Bachelor's or Master's degree in psychology, social work, counseling, or a related field.
      • Licensure or certification as a mental health case worker, counselor, or therapist, as required by state regulations or the specific PRP program.
      • Solid understanding of mental health disorders, evidence-based therapeutic approaches, and trauma-informed care.
      • Excellent communication skills, both written and verbal, for effective client interaction and reporting.
