Overview

POSITIONAL SUMMARY:  

Location: Palo Alto

The Wellness Coordinator is responsible for overseeing and coordinating the comprehensive applicable care management of patients on a 360° level. This role serves as a key point of contact for patients with applicable conditions, working closely with healthcare providers to optimize their care, enhance patient outcomes, and improve overall quality of life. The Wellness Coordinator plays a vital role in coordinating and implementing personalized care plans, providing ongoing support, and facilitating effective communication between patients, caregivers, providers, and other healthcare professionals. 

Ideal candidate must have a positive and supportive attitude toward the agency and its overall mission. Ideal candidate must have the ability to think globally, solve problems and have good decision-making skills, including critical thinking skills.

Compensation - starting at $21/hr based on experience

Schedule - between 8am and 7pm

DUTIES AND RESPONSIBILITIES: 

Positional responsibilities include, but are not limited to:

  • Company representative housed in a senior living community providing support to residents, staff, families, and providers.
  • Take monthly vitals and weights on community residents.
  • Support in the care and coordination and communication of services for residents.
  • Support residents with telehealth appointments and technology.
  • Provide daily wellness checks on company program enrolled programs.
  • Provide companionship to community residents.
  • Participate in community and company events.
  • Communicate critical findings to appropriate contacts, as needed.
  • Provide education and support to patients and their families on managing their applicable conditions, including self-care techniques, symptom management, and resources available. 
  • Facilitate effective communication among patients, healthcare providers, and other members of the care team to ensure seamless coordination of care. 
  • Participate in quality improvement initiatives, collaborating with the healthcare team to enhance patient quality of life.
  • Assists lead or supervisor in orienting, training, and mentoring staff. 
  • Provides ongoing training to staff as needed. 
  • Additional responsibilities, as assigned.

MINIMUM QUALIFICATIONS: 

  • At least 18 years of age
  • High school diploma, GED, or equivalent 
  • Medical Assistant Diploma, preferred 
  • Education or experience in patient care, preferred
  • Education or experience in case management, preferred

PERFORMANCE MEASUREMENT: 

Performance will be measured based on the implementation of the following:

  • Job description roles and responsibilities.

  • Knowledge, understanding and implementation of departmental objectives and company core values.
  • Maintains professionalism and excellent internal and external customer service standards.
  • Adherence and encouragement to others regarding safety and health compliance standards.
  • Communication with medical professionals, vendors, families, peers, and supervisors. 
  • Building positive relationships with clients, families, peers, and supervisors.
  • Actively creates a positive, respectful, and inclusive working environment.

WORK ENVIRONMENT: 

This position requires regular sitting, stooping, reaching, walking, standing, reading, seeing, speaking, hearing, listening, organizing, interpreting data and information, operating office equipment, typing using a computer keyboard, and viewing a computer screen monitor, and occasional lifting of objects of up to 50 pounds (if applicable).

HIPAA – Complies with the Health Insurance Portability and Accountability Act (HIPAA) and ensures company policies and procedures pertaining to HIPAA are being followed to properly manage client information.

Teamwork - Cooperates and works constructively with others to accomplish common goals and objectives and treats others with respect and dignity by valuing the contributions of everyone.

Initiative - Proactively identifies ways to contribute towards the goals and objectives of the company and takes proper action to address issues and opportunities. 

Task Management - Budgets, schedules, and manages own time in order to fulfill priorities effectively and efficiently, while also ensuring proper allocation of workload and duties for staff.
