This will be a full-time role (40 hours/week), with a flexible schedule across Sunday through Saturday.
Job Summary
The Nurse Coordinator Developmental Opportunities Services provides health care oversight and medical care management for an identified caseload of youth in compliance with Office of People with Developmental Disabilities regulatory requirements and agency policy and procedures. 

Essential Job functions

  • Manage coordinated Health Services which involves designing a plan for delivering healthcare that aligns with accepted community standards. This plan should consider the unique needs of the individuals served by the program and should involve collaboration among various healthcare professionals.
  • Adhere to professional standards ensures quality care delivery and maintains ethical and legal integrity in healthcare provision. This includes staying updated on best practices, regulations, and guidelines relevant to the field.
  • Work within an interdisciplinary team using a holistic approach to healthcare to address the diverse needs of the individuals in the program.
  • Provide medical oversight to ensure that healthcare services are delivered safely and effectively. This may include monitoring the health status of individuals, managing medications, responding to medical emergencies, and coordinating with external healthcare providers when necessary.
  • Adhere to regulations set forth by the Office for People With Developmental Disabilities (OPWDD) ensures that the program operates legally and ethically. This includes following protocols for documentation, reporting, and maintaining a safe and supportive environment for individuals in care.
  • Provide high-quality nursing and medical care within the framework of professional standards and regulatory requirements, while also promoting the well-being and independence of the individuals served by the program.
  • Document in each participant’s electronic clinical record all diagnostic assessments, individual plans of treatment, and treatment notes regarding recommended services and appropriate treatment.
  • Maintain and adhere to ethics of confidentiality. Discuss confidential matters in an appropriate manner in compliance with the Health Insurance Portability and Accountability Act (HIPAA)
  • Provide education to staff and participants on relevant consumer health topics to ensure proper care and follow-up.
  • Advocate on behalf of youth/individuals to establish appropriate health care. 
  • Consult with outside providers or other members of the participant’s health care team to follow-up on physical or psychiatric medical concerns, and implement follow-up care recommendations. 
  • Ensure that all referrals are documented accurately and promptly. This includes gathering necessary information about the individual being referred, their healthcare needs, and any relevant background information. Follow up with the referral source to confirm receipt of the referral and to provide any additional information that may be needed.
  • Attend admissions meetings to obtain medical/psychiatric history and determine individualized health care needs. 
  • Complete prior authorization for medications or medical necessity based on recommendation from medical and psychiatric team.
  • Develop the discharge summary establishing health care follow-up.
  • Provide the clinical functions of a Registered Nurse (RN) within the scope of their practice in NYS which is defined as: Diagnosing and treating human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, and provision of care supportive to or restorative of life and well-being, and executing medical regimens prescribed by a licensed physician, dentist or other licensed health care provider legally authorized under this title and in accordance with the commissioner's regulations. A nursing regimen shall be consistent with and shall not vary any existing medical regimen.

Education & Experience

Successful completion of an accredited Associates Degree Nursing Program and 2 years experience as an RN.

Special Requirements:

  • Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.
  • Active, unrestricted RN license in NYS required
  • Basic Life Support (BLS) training within 60 days of hire and recertification as necessary..

Being proactive in addressing these issues can prevent escalation and improve outcomes.
  • Knowledge of regulatory and practice regulations: Understanding and adhering to regulatory requirements, such as those outlined by HIPAA, ensures compliance and protects the privacy and rights of individuals. This includes knowledge of treatment protocols and best practices in healthcare delivery.
  • Continuous learning: Healthcare is a dynamic field, so the ability to stay updated on new information, research, and practices is crucial. This includes actively seeking out opportunities for education and professional development to enhance knowledge and skills relevant to the role.
  • Demonstrate computer proficiency: In today's digital age, proficiency with computer-based applications is essential for managing data, documentation, and communication efficiently. This includes using electronic health records (EHR) systems, scheduling software, and other relevant tools.
  • Knowledge of available services: Understanding the range of services provided both internally by the agency and externally by other service providers allows for effective coordination of care. Maintaining contacts and relationships with external providers ensures timely access to resources and support for clients.

  • Must be able to work a variable scheduling including evenings and weekends
  • The following physical demands are required:
    • Occasional sitting (up to 4 hours)
    • Occasional standing (up to 1 hour)

    • Continuous balancing (up to 8 hours)
    • Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours)
    • Weekly lifting up to 50 lbs. (community-based staff may be required to lift an empty wheelchair weighing up to 50 lbs.), 10-15 lbs. on a daily basis
    • Weekly carrying up to 55 lbs. approximately 10-100 ft., 10-15 lbs. on a daily basis
  • Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)
  • Occasional exposure to dust, fumes, gases or chemicals is apparent. 
  • Protective clothing or equipment as required: Close toed shoes; Protective Shields, Universal Precaution PPE, Gloves for toileting and food preparation
  • Ability to change positions as needed

$ 52,208.00 Minimum Rate per year, $ 82,446.00 Maximum Rate per year, based on experience.
