Work Schedule

This position is casual (at least 3 days per week) 8am to 4:30pm plus weekend and call rotation as needed

Qualifications for the Home Care LPN

• High school diploma or equivalent
• Valid Michigan LPN license
• Minimum of one year of current, continuous experience in an acute care setting is preferred
• Must have a dependable vehicle with current driver's license and auto insurance coverage
• BLS and ACLS certification (available through Hillsdale Hospital)
• Proficient with computer software used in healthcare

Responsibilities of the Home Care LPN

• Calls patients to schedule visits
• Evaluates patients' and residents' health and implements plan of care in collaboration with the patient, family, and physician
• Administers treatment, medications and IV solutions according to physician’s orders
• Observes and evaluates symptoms and reactions to treatment; initiates corrective action if needed, updating plan of care and advising physician of changes in patient's condition
• Documents and maintains records and reports regarding nursing care provided 
• Completes OASIS information and performs the comprehensive assessment as SOC, resumption, transfer, death in the home and at discharge
• Coordinates services to facilitate holistic patient care
• Maintains daily visit productivity of 5 to 7 visits adjusted for travel and other assignments
