Hillsboro Medical Center is Hiring a Clinical Pharmacist - Ambulatory Care Near Hillsboro, OR

POSITION SUMMARY

Pay range: $133,244.80/year - $199,867.20/yr

The Clinical Pharmacist- Ambulatory Care is responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients. In addition, will serve as a resource to the pharmacy, nursing and medical staff within the field of primary care drug management.

KEY RESPONSIBILITIES

Performed majority of the time:

· Provides collaborative drug therapy management to patients in the primary care clinic to include:

o Initiation and dose adjustment of medications for conditions outlined in the Physician/APP orders.

o Patient and family education.

o Drug/drug interaction monitoring.

o Drug/food interaction monitoring.

o Follow-up instructions.

o Coordination with diabetes education, nutrition, home health services and other appropriate resources as applicable to assure optimum patient outcomes.

o Assist in appropriate patient screenings and follow-up for depression and substance use.

o Appropriately documents all clinical activities.

o Acquires any needed prior authorizations.

· Coordinates pharmaceutical care across all ambulatory care clinics:

o Coordinates medication-therapy related quality initiatives.

o Develops clinical workflows.

o Develops and manages outpatient medication formulary for primary care clinics.

· Participates in the training of practitioners, pharmacists, other healthcare professionals, and learners (including pharmacy and medical students/residents); preceptor for pharmacy residency program.

Performed occasionally but critical to successful performance of the job:

· Serves as a resource to the pharmacy, nursing, and medical staffs in primary care.

Decision making and budget responsibilities (e.g. impact limited to the employee, entire unit or organization-wide):

· Impact limited to the employee and care of individual patients

· No budget responsibility

JOB SPECIFICATIONS

Education:

Required

· Pharm D.

Preferred

· N/A

Experience:

Required

· One of the following:

o Completion of a PGY-1 residency plus 2 years of ambulatory care experience.

o Completion of a PGY-2 ambulatory care residency.

o 5 years of experience in ambulatory pharmacy care.

Preferred

· Comprehensive clinical pharmacy knowledge base with emphasis in the field of depression and substance use.

Licenses, Certifications and/or Registrations:

Required

· Current, valid pharmacy license by the Oregon Board of Pharmacy.

· BLS required within 90-days of hire.

· ACLS required within 90-days of hire.

Preferred

· Active board-certified ambulatory care pharmacy specialty certification.

· Comprehensive clinical pharmacy knowledge base with emphasis in the field of diabetes and hypertension.

· Knowledge of and understands the current guidelines for managing medications, associated risks, and patient assessment.

· Demonstrated ability to communicate effectively in verbal and written form with the healthcare team and with patients.

· Demonstrate ability to effectively evaluate and adjust patient specific medication therapy.

· Demonstrate ability to prioritize multiple responsibilities in a group of ever growing, busy clinics, dividing time accordingly.

· Demonstrate ability to effectively develop an ongoing relationship with the clinic patients in order to assure optimum outcomes.

· Demonstrate ability to effectively and completely document all patient and physician communications and interactions.

· Demonstrate ability to communicate effectively with patients of varying social, educational and cultural backgrounds.

· Must be able to work independently with no direct supervision, set priorities, make critical decisions, respond quickly to needs, and adapt to changes in workload.

Preferred

· Bilingual skills a plus

· Comprehensive clinical pharmacy knowledge base with emphasis in the field of depression and substance use.
