Your Responsibilities

As a School/Home Therapy Coordinator, you will coordinate the services provided in the SBYS and BHIS Programs, including but not limited to: supervising staff, liaising with the school administration, coordinating meetings with other school officials and staff, working with families and provide direct service up to 50% on average. Responsibilities will also include:

  • Completing an initial assessment, diagnostic summary, and other assessments as required
  • Provide consultation to School Based Youth Services counselors
  • Provide treatment coordination services to school personnel (school liaison)
  • Assist with implementation of t-MHFA training program with local high schools
  • Participation in trainings as requested by Hillcrest Family Services
  • Conduct individual and group counseling to SBYS youth with Counseling as a part of their IEP
  • Display accurate assessment and diagnostic skills
  • Work on processes and procedures with Hillcrest Leadership.
  • Provide LPHA assessments for BHIS clients.
  • Comply with all Company rules and procedures related to medical records, confidentiality, supervisor, professional, billing, documentation, HIPAA, consent of minors, relationship with parents.

What You Need

  • Masters degree in counseling, social work or closely related field
  • Licensed Mental Health Counselor or Licensed Independent Social Worker preferred
  • Iowa licensed to practice or eligible 
  • Effective counseling and diagnosing skills, and a proficient knowledge of the impact of trauma
  • Proficiency in time management and EHR documentation
  • Excellent communication and interpersonal skills. Ability to work cooperatively with staff and other service providers and interface effectively, sensitively with students/families
  • Ability to demonstrate the knowledge and skills necessary to provide services appropriate to the age and diagnosis of students served
  • Highly professional presentation and manner consistent with school standards and Hillcrest policies
  • Ability to assist school staff to identify effective strategies to de-escalate or avoid triggering a student
  • Must be able to perform the essential functions of this position
  • Willingness to drive to different locations within the community 
  • Valid driver's license

About the Program

The School-Based Youth Services Program provides brain health services directly to students within the school setting during regular school hours. Students are referred by school counselors. School based social emotional behavior counselors work in collaboration with school faculty, as well as parents or guardians to coordinate care and treatment of the student. Services are provided throughout multiple schools within the Dubuque Community School District. These services include:

  • Outpatient brain health services within the school setting
  • Screening, assessment, and diagnosis
  • Patient-centered treatment planning
  • Referring to additional community resources and services when needed

The BHIS Program works with families to help create supportive environments at home. They offer the tools, training, and support that address the behavioral health needs of children in the home. The School/Home Therapy Coordinator works with families in the privacy of their home to ensure comfort while learning new skills. The School/Home Therapy Coordinator engages the youth in skill-building programs that target specific behavioral struggles and help to improve a childs skills. The School/Home Therapy Coordinator works to empower children and help them manage their behaviors effectively. The School/Home Therapy Coordinator work to build skills in the areas of communication, coping with difficult emotions, managing anger, conflict resolution, and decision making.
