Hill View Retirement Center is Hiring a Licensed Practical Nurse - Day Shift - Lakeside Near Portsmouth, OH

 

SUMMARY:

Primarily responsible for providing nursing care to residents in Assisted Living and Wesley Village in accordance with facility policies and procedures. Responsible for monitoring residents, to identify care needs, and to take action to assure these needs are met. Responsible for supervising and coordinating nursing care delivery of Assisted Living staff on his/her assigned shift. When assigned to Wesley Village, responsible for responding to resident health care needs in Independent Living areas during times when the Clinic is closed.

ESSENTIAL DUTIES AND RESPONSIBLITIES:

Performs resident health assessment upon admission, and annually, and documents the assessment on the standardized assessment form.

Administers medication as scheduled on the Medication Administration Record (MAR). Documents medications on the MAR.

Performs nursing treatments in accordance with established policies/procedures. Documents treatments and resident’s response.

Monitors resident lab reports. Communicates with physician about abnormal results.

Receives and gives concise, accurate report on residents at beginning and end of each shift.

Monitors residents for possible adverse reactions to medications and treatments. Reports adverse reactions immediately to the physician, Director of Nursing and Assistant Director of Nursing

Monitors residents daily for signs of infections, skin problems, fluid and dietary needs. Notifies the resident’s physician of problems.

Checks resident medication supplies daily. Orders medications from local or mail-order pharmacy as needed.

Monitors and documents resident vital signs as ordered, and prn. Communicates with physician about abnormal findings.

Monitors and documents resident weights as ordered, and prn. Communicates with physician about abnormal findings.

Receives, transcribes and notes physician orders in accordance with facility policies/procedures.

Monitors and records resident falls and other incidents in accordance with facility policies. Communicates with physician, Director of Nursing, Assistant Director of Nursing and other nursing staff about these incidents.

Documents episodic events of individual residents in nursing notes, including new medication administration, change in condition, etc.

Communicates with physician and family members when change in resident condition or resident transfer occurs.

Gives report to receiving unit/facility nursing staff when residents are transferred from Assisted Living. Completes appropriate paper work.

Prepares C.N.A.’s assignments each shift. Supervises C.N.A.’s to assure that services and resident care are being delivered in accordance with facility standards.

When working in Wesley Village, responds to Independent Living resident emergency calls immediately when the Clinic Nurse is not working. Provides nursing support to Independent Living residents in their apartments. Documents nursing care in the Clinic record and on the Wesley Village communication clipboard. Notifies Clinic nurse via

e-mail of the call and actions taken.

When working in Wesley Village, monitors Independent Resident absence list on days when the Clinic is closed. Follows up with absent residents, to determine that they are safe.

When working in Wesley Village, communicates disaster warnings/information to Independent Living residents when the office is closed. Communicates via the general intercom system. Coordinates resident assistance as needed.

Assists C.N.A.’s with resident care issues as needed.

Reviews and knows definition of pager codes.

Answers telephone as needed, using proper telephone etiquette.

Communicates with designated departments via e-mail system each shift, about resident census, transfers, and admissions/discharges/deaths.

Communicates via e-mail system with Director of Nursing and Assistant Director of Nursing about changes in resident conditions. Contacts Director of Nursing and Assistant Director of Nursing immediately about significant changes in resident condition.

Communicates with Director of Nursing and Assistant Director of Nursing about equipment needs, and safety concerns.

OTHER JOB DUTIES:

Complies with all company policies/procedures related to resident privacy/ confidentiality and information system security, to insure compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

Observes facility policies and procedures, including Resident Rights policies.

When working in Wesley Village, receives employee injury reports during hours that the offices are closed. Coordinates post-accident drug/alcohol testing with the local hospital when employee requires medical treatment. Communicates with Human Resources Director via voice mail or e-mail about the injury.

Performs job duties in an efficient, productive, and professional manner.

Actively supports the implementation of Hill View’s mission and statement of purpose.

Participates in training, workshops, etc. to keep abreast of current issues/trends related to his/her job duties.

Accepts job assignments with positive spoken word and body language.

Performs other duties as assigned.

SUPERVISORY RESPONSIBILITIES: Supervises daily operations and performance of Resident Helpers on the assigned shift.

QUALIFICATIONS: Actively licensed as a Practical Nurse or Registered Nurse in the State of Ohio. Six months of clinical experience required. Strong leadership, interpersonal and organizational skills required. Previous long term care experience preferred.

EDUCATION AND/OR EXPERIENCE: Must be a graduate of an approved LPN or RN program.Must be able to read and interpret policies and procedures. Must have the ability to respond to inquiries made by staff, managers, physicians, residents, families and regulatory agencies.

MATHEMATICAL SKILLS: Must have ability to add, subtract, multiply, and divide all units of measure using whole numbers, common fractions and decimals.

REASONING ABILITY: Must have ability to apply common sense understanding to carry out detailed, but uninvolved written or oral instructions. Must have ability to solve practical problems and concrete variables in situations where only limited standardization exists.

PHYSICAL REQUIREMENTS: Frequently required to stand, walk, sit, use fingers, handle or feel, reach with hands and arms, climb or balance. Must be able to speak clearly and hear with minimal or no assistance in order to perform physical evaluations. The employee must frequently lift and/or move up to 50 pounds and occasionally lift/move more than 100 pounds. Specific vision abilities required in this job include close vision, distance vision, peripheral vision, depth perception, differentiation in color, and ability to focus.

WORK ENVIRONMENT: The indoor environment is standard with fluorescent lighting and central heating/cooling. While performing this job, the employee may be subjected to hostile or emotionally upset residents/visitors. The employee may frequently be required to use protective equipment to prevent exposure to blood and body fluids. The employee may encounter frequent interruptions.
