Help us help others:

This position is responsible for completing PASRR Evaluations in Hays and Comal counties, as well as tracking PASRR Evaluation assignment and completion in HC-MHDD’s catchment area. Provides oversight in collaboration with Mental Health Clinic Directors, PASRR direct service providers, and IDD service coordination. Other management tasks associated with PASRR responsibilities are assessing, evaluating, and identifying needs and goals of people with mental health needs relating to their personal recovery while residing in a nursing facility. This position is responsible for the development and ongoing training for staff specific to PASRR. The PASRR Program Specialist is responsible for providing activities in accordance with state and federal rules. The PASRR Program Specialist adheres to strict timelines per the TAC Title 26, part 1, Chapter 303 and all related paperwork must be completed using the CARE, LTC (Long-Term Care) Online Portal/ TMHP (Texas Medicaid & Healthcare Partnership), and the agency’s EHR system. Requires flexibility in schedule and travel to meet the needs of the program.

You will get to:

  • Complete all PASRR Evaluations in Hays and Comal Counties accurately and within required timeframes.
  • Respond to all alerts from LTC (Long-Term Care) Online Portal/ TMHP (Texas Medicaid & Healthcare Partnership) within required timeframes.
  • Develops and maintains a tracking system to meet the HHSC contract measures.
  • Responds to all PL-1 (Pre-admission screening Level 1) alerts from the community as required by TMHP/HHSC designed to validate the suspected diagnosis of Mental Illness (MI).
  • Work with Mental Health Clinic Directors to ensure that all required timeframes set forth by regulatory agencies are met.
  • Maintains compliance with Texas Administrative Codes (TAC) and HHSC performance contract.
  • Ensures required outcome/target goals of agency and performance contract measures and targets are met.
  • Direct entry in CARE / Data entry and database management.
  • Provides support, leadership, and training to staff.
  • Works under general supervision with moderate latitude for the use of initiative and independent judgment.
  • Collaborate with nursing facilities and Mental Health Clinic staff to link the individual with outcome-focused services within the required timeframe.
  • Attends work regularly and as scheduled.
  • Works cooperatively with others and the supervisor.
  • Communicates effectively and in a timely manner with team and individuals served and/or their legally authorized representative.
  • Follow the agency’s workplace values and principles.
  • Maintain client confidently and privacy according to standards.
  • Follow policy and procedures for protecting clients as well as reporting client abuse and neglect if needed.
  • Protect and ensure client rights always.

Requirements


You’ll definitely need:

  • Bachelor’s degree from an accredited college or university in social, behavioral, or human services
  • Minimum one-year direct client care experience in mental health.
  • Experience working within a team setting.
  • 3-5 years of experience working in a mental health setting providing community-based services within the community (Preferred)
  • Bilingual: English/Spanish (Preferred)
  • To maintain a valid Texas State Driver’s License.
  • To have current automobile insurance and the ability to travel to work locations frequently.

  • Adhere to all Hill Country MHDD Centers policies and procedures.
  • Maintains ANSA/CANS certification.
  • Travel and driving agency or personal vehicle as required.

  • This position attends work on a regular and predictable schedule in accordance with Hill Country MHDD Centers policies
  • Complete all PASRR Evaluations in Hays and Comal Counties accurately and within required timeframes.
  • Assist other counties with completion of PASRR Evaluations as needed.
  • Assess/Evaluate designated residents for eligibility.
  • Employ excellent verbal and written communication skills to engage with team.
  • Maintain a well-organized calendar, files, and prioritize effectively ensure completion of duties.
  • Skill in managing competing and changing priorities.
  • Maintain accurate and timely documentation and with a high level of typing and communication proficiency.
  • Attention to detail – must possess the ability to maintain high accuracy and thoroughness when executing tasks.
  • Sustain a calm demeanor when under pressure to handle sometimes difficult situations.
  • Provide exemplary customer service.
  • Work cooperatively with others and be punctual and consistent in attendance because your people will rely on you!
  • Work autonomously to complete assignments.
  • Represent HCMHDDC in the community as a responsible resource.
  • Other duties as assigned.

$48,500 - $56,000 a year with benefits eligibility plus good karma for doing good in the world
