Patient Resource Navigator

Program/Location: Plymouth Multi-Service Center/ Plymouth
Education/Licensure: Bachelors degree in related field or equivalent experience
Pay Rate: $21 - $25 per hour
Status: Full time
Shifts: Tuesday-Friday 11:30am-8:00pm; Saturdays 9:00am-5:00pm

About Us

High Point Treatment Center is excited to be part of the state’s expansion of behavioral health services in the Brockton area. The Community Behavioral Health Center (CBHC) located at the Brockton Meadowbrook Campus has opportunities for growth within the outpatient, community based, and Adult Crisis Stabilization programs. We are seeking enthusiastic change makers, who are looking to grow with the new services.

High Point & Affiliated Organizations is a health and human service agency whose mission is to treat and prevent substance use disorders and mental illness. High Point has programs located throughout Southeastern Massachusetts offering a full continuum of care for substance use and mental health treatment, including inpatient, outpatient, residential, and community-based services. Programs and services also assist survivors of abuse, violence, and families experiencing homelessness. High Point believes that everyone has inherent goodness, worth, and dignity. Patient Resource Navigator Qualifications

  • Bachelor’s Degree level degree in Psychology or a related human services field (preferred), or equivalent experience
  • Bi-lingual/Multi-lingual applicants are strongly encouraged to apply
  • Excellent interpersonal and communication skills while able maintaining professional boundaries
  • Maintain mental alertness and attention to high detail
  • Maintain a neat and professional appearance and provide excellent customer service as needed
  • Follow established security protocols while using initiative and good judgement without bias

Patient Resource Navigator Duties & Responsibilities

  • Help to support an open access model for screening and assessments for mental health, substance use, and co-occurring treatment services within the CBHC service model
  • Provide initial screenings and assist patients and families with accessing CBHC services
  • Assist patient with intake paperwork and ensure proper release and consent forms are complete
  • Participates in patient outreach and engagement to support access to treatment
  • Coordinates patient transportation services, as needed
  • Provide care coordination and discharge planning activities to support transitions of care
  • Maintain and document collateral contacts with other providers as needed, including, but not limited to primary care physicians, probation, parole, DCF, family members, natural supports, etc
  • Coordinate with local Emergency Services Provider for concerns related to imminent psychiatric risk and hospitalization, referral support for CBHC core services, and follow-up
  • Engage in community outreach activities as needed
  • Perform other duties as assigned; including providing security support for the program to monitor patient and staff safety as part of their duties

Patient Resource Navigator Requirements

  • Must present as courteous and professional at all times
  • Demonstrated flexibility and ability to perform multiple tasks
  • Must have excellent communication skills, including the ability to organize ideas in logical and clear fashion
  • Must have the ability to work with others in cooperative and collaborative manner
  • TB screening
  • Certification in the HPTC approved de-escalation program
  • Strong and effective verbal, written and organizational skills
  • Knowledge of basic computer skills and software programs including the ability to learn, use and train HPTC`s electronic medical records
  • Ability to provide leadership and program development within an outpatient site
  • Knowledge of major clinical therapeutic models and behavioral health best practice
  • Strong organizational, verbal and written communication skills
