TITLE:Residential ARMHS Practitioner

PROGRAM:Residential Services

JOB SUMMARY:To coordinate and provide direct services in the community to adults with a diagnosis of serious and persistent mental illness. Responsible for ensuring the individual receives services as outlined in a diagnostic assessment to enable them to live as independently as possible in the community. Educate and train individuals on the basic skills necessary to enable them to live in the least restrictive setting.

JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:

  • Manage a caseload of clients experiencing mental health symptoms, meeting with each individual as determined by their level of need. Caseload may involve only those in the residential or HBL settings, depending on ARMHS assignment. 
  • Teach clients basic skills such as budgeting, independent living skills, handling crisis, coping with symptoms of their mental illness, handling benefits, and developing social skills. 
  • Must be able to support clients during crisis situations utilizing verbal de-escalation and grounding techniques.
  • Offer referrals and recommendations for community resources and services.
  • Provide transportation or arrange transportation for individuals as needed.
  • Understand standards and follow time lines for handling referrals, and meeting with the client. 
  • Maintain case files and appropriate case notes and billing as mandated by agency and state protocols.
  • Coordinate with other providers assigned to working with the individual at a minimum of every 6 months.
  • Participate in clinical supervision individually and in-group once a month or more if licensure requires.
  • Attend required organization or community meetings.
  • Attend conferences, in-services, and training necessary to meet licensure and program requirements.
  • Participate in Residential Services weekend emergency on-call rotation as assigned.

PHYSICAL REQUIREMENTS FOR POSITION:Must be able to walk, sit, stand, crouch, twist, bend, reach overhead, maneuver stairs and stretch in a manner conducive to the execution of daily activities. Must be able to move around the office and community settings on a daily basis. While performing the duties of this job, the employee must communicate with others and exchange information. Employee regularly operates a computer and other office equipment on a daily basis. Occasional bending and lifting up to 30 pounds.

NON-ESSENTIAL FUNCTIONS: Perform other duties as assigned by the Program Director.

EQUIPMENT USED: Computer, telephone, office equipment, vehicle.

JOB QUALIFICATIONS AND REQUIREMENTS:

  • Must meet minimum qualifications as a Mental Health Practitioner as outlined in MN Statute 245i.04: Sec. 245I.04 MN Statutes
  • Possess a high level of human relation skills and the ability to work with and react appropriately to a variety of people in difficult or challenging situations. Ability to calmly and repetitively model appropriate behavior in difficult or challenging situations. 
  • Skilled in planning, problem solving and decision-making. Must be highly organized and creative with an ability to locate resources for clients as needed. 
  • Exceptional organization and communication skills (oral and written), must be able to demonstrate an ability to work with a team. 
  • Must be able to maintain confidentiality.
  • Have skills and knowledge acquired through at least one of the following: 
  • a course of study in a health or human services related field leading to a bachelor of arts, bachelor of science, or associate’s degree; 
  • one year of experience with the target population served; 
  • experience as a certified peer specialist according to Minnesota Statutes, section 256B.0615; or 
  • meet the requirements for unlicensed personnel under Minnesota Statutes, sections 144A.43 to 144A.483.
  • Must possess a vehicle valid driver’s license and a willingness to travel as needed to client or organization locations throughout SE MN.
  • Must hold a current certificate in CPR/AED/First Aid or be willing to obtain.

POSITION DESIGNATION: Non-exempt, Full-time or Part-Time

BENEFIT SUMMARY:

  • Paid Leave Time starts accruing at 5.08 hours per pay check (pro-rated for employees under Full-Time).

  • 403b retirement account with employer contribution of 100 percent match, up to 3.5 percent, at one year (1040 hours). Employees are fully vested at one year.
