Position Title: Wellness Director Department: Nursing
The Employee shall use his/her best efforts to provide the highest level of professional service; conduct
him/herself at all times with a pleasant and professional demeanor; and maintain and improve his/her job
skills. Employee agrees at all times to perform his/her duties faithfully, industriously, and to the best of
his/her ability, experience, and talent, to the satisfaction of Employer. The Employee’s duties shall include,
but not be limited to the following:
Scope:
Works with all departments as well as outside agencies.
General Function:
To oversee the Personal Care-Assisted Living Wing. Multi-task Personal Assistants.
Duties and Responsibilities:
• Performing ADL/IADL evaluations at least on a quarterly basis;
• Making home visits to evaluate potential residents;
• Assisting with the admission paperwork;
• Insuring all paperwork is completed during the required time frame;
• Giving TB’s to new residents and annually thereafter;
• Insuring residents receive annual physicals, annual level of care certification,
annual self administration of med certification, and are seen by their physicians at
least every six months;
• Check-in medications;
• Monitor medications;
• Set-up medications weekly;
• Document on residents when condition changes or at least quarterly;
• Maintain a monthly MAR for each resident;
• Thin resident charts;
• Audit resident charts;
• Perform evaluations when needed;
• Answer questions by family and residents;
• Orient new residents to the center;
• Orient family members to the center and answer family member questions;
• Supervise Assisted Living staff;
• Make schedules for Personal Care Attendant staff;
• Interview and hire Assisted Living staff under the direction of the Director of
Nursing;
• Train new Assisted Living staff;
• Insure wing is clean, neat and orderly;
• Dispense medication when necessary;
• Work with the local pharmacy on resident forms needed for the center;
• Review procedures and offer suggestions to improve quality of care;
• Assisting with routine patient care (bathing, dressing, grooming, and setting-up
medications for the residents);
• Assisting when needed: cleaning the personal care wing hallways and resident
rooms, serving the resident meals, providing activities, and other duties needed to
be done to ensure the wing is running effectively and efficiently;
• Is on call to answer questions from staff or for finding a replacement if there is a
call-off;
• Other assigned duties by the Director of Nursing;
• Providing weekly Wellness Director Report and Tracking sheet and resident
report to Director of Nursing;
• Provides quarterly QA report to Director of Nursing and presents findings to QA
committee;
• Monitors and ensure annual dietary reviews are completed;
• Ensure monthly weights are obtained for residents;
• Provide copy of weekly resident report to medical nursing station;
• Working hours 8:00AM – 4:30pm with every other weekend and every other
holiday off when staffing permits.
Qualifications:
Must be a Licensed Practical or Registered Nurse and must have a current license for the
state of Ohio. One year experience preferred in the geriatric setting. Must be organized
and can handle many different tasks at the same time.
Because of close contact with the residents, this position has been determined to have
potential for exposure to blood borne pathogens. Country Club Rehabilitation Center
will provide training, information and equipment to minimize the potential risk.
It is a requirement of this position that the individual comply with all privacy policies and
procedures of the Provider and protect the privacy of Personal Health Information as
required by law.
Employee shall not have the right to make any contracts or commitments for or on behalf of Employer
without Employer’s prior consent. Although the parties hope to maintain a long and mutually beneficial
relationship, abiding by this job description does not guarantee continued employment. Employment is AT
WILL. Either party may terminate the employment relationship between the parties, for cause or no cause.
