Duties
- Conduct comprehensive assessments of patients' social and emotional needs
- Collaborate with healthcare team to develop and implement individualized care plans
- Provide counseling and support to patients and their families regarding medical conditions and treatment options
- Assist patients in accessing community resources and support services
- Advocate for patients' rights and ensure they receive appropriate care and services
- Document patient interactions, assessments, and interventions in accordance with regulatory standards
- Stay up-to-date on medical terminology, diagnostic evaluations, and treatment modalities
Requirements
- Bachelor's or Master's degree in Social Work or related field
- Valid licensure as a Medical Social Worker (LMSW) or Clinical Social Worker (LCSW)
- Knowledge of data collection methods and ability to analyze and interpret data
- Familiarity with HIPAA regulations and ability to maintain patient confidentiality
- Experience in child and family counseling, including working with individuals with autism or special education needs
- Strong assessment skills to evaluate patient needs and develop appropriate care plans
- Ability to provide clinical counseling to individuals with complex medical conditions
- Knowledge of dementia care principles and techniques
