Heritage Home Health and Hospice is Hiring a Social Worker (PRN) Near Tyler, TX
Heritage Home Health is currently looking for a Part time License Social Worker. Must have a current social worker's license in the state of Texas, all others need not apply. Bachelors degree required. The Primary function is to provide coordinated care in the home to hospice patients of all age groups and perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the patient/family. The position is PRN. The ideal candidate would possess experience in the hospice setting, able to travel locally in Tyler Tx and surrounding areas, and an individual of integrity. Pay is per visit rate and is competitive. Please apply though Indeed.
