Position Type: Full-Time or Part-Time (Hybrid)

Reports To: ECM Director

Salary: $85,000 - $110,000 annual / $40 hr - $46 hr

Role Overview:

The Behavioral Health Care Manager (BHCM) is a master’s level professional responsible for the management and coordination of behavioral health services within HHN's ECM program. This role is crucial for addressing the complex behavioral health needs of our members, and ensuring they receive comprehensive, culturally sensitive, and empathetic care.

Key Responsibilities:

  • Behavioral Health Assessments: Conduct comprehensive assessments to identify behavioral health needs and develop individualized care plans.
  • Care Management: Provide ongoing management of behavioral health conditions, including counseling, crisis intervention, and support services.
  • Integrated Care Coordination: Work closely with the ECM care team to ensure a holistic approach to member care, integrating behavioral health with other medical and social services.
  • Progress Monitoring: Regularly monitor and evaluate member progress, adjusting care plans as necessary to meet evolving needs.
  • Resource Liaison: This position serves as a key liaison between members, their families, and external behavioral health resources, facilitating access to necessary services.

Qualifications:

  • Master’s degree in Social Work, Psychology, Counseling, or a related field (i.e., ASW,
  • LCSW, LMFT, AMFT, LPCC, MSW, APCC
  • Valid Driver's License
  • Personal connection to and understanding of the health disparities affecting the Black community and other vulnerable populations.
  • Demonstrated experience in behavioral health care management, with a strong foundation in counseling, crisis intervention, and care coordination.
  • Exceptional interpersonal, communication, and collaborative skills, with the ability to work effectively within a multidisciplinary team.
  • Deep understanding of Medi-Cal regulations, behavioral health care practices, and the unique needs of vulnerable populations.
  • A commitment to providing empathetic, culturally competent, and patient-centered care.
