Henry Ford Health System is Hiring a Pharmacy Specialist (Palliative Care) - 40 hrs/wk - Day Shift Near Detroit, MI

***Postgraduate Year One Pharmacy (PGY1) Residency training needed, preferable completion of Postgraduate Year Two (PGY2) Pain Management and Palliative Care Pharmacy Residency or equivalent pharmacy practice experience***

Job Duties and Responsibilities:

· Review medication profiles for patients receiving palliative medicine consultation

o Complete comprehensive medication history and review of medication profile

o Evaluate medication regimen for drug-drug and/or drug-disease interactions

o Review and interrupt lab results, this includes ordering of appropriate labs needed for patient care

o Monitor patients for development of adverse drug events including mitigation strategies for adverse drug events

o Provide dosing recommendations for patients, this includes assessing renal, hepatic and cardiac history

o Establish goals of care with providers and assist with educating patient and caregivers on medication therapy decisions

o Educate patients and caregivers regarding medications

· Serve a resource to the medical, nursing and pharmacy team regarding safe and effective management of medications used in the treatment of pain and related symptoms

· Participate in collaborative rounds for inpatients on the palliative medicine service

· Provide clinical consultations or recommendations to the palliative medicine team on pain management that includes dosing, symptom management, cost - effective medication therapy management, palliative sedation and transitions of care

· Respond to drug information questions from providers, nursing and administrators pertaining to palliative medications

· Oversee medication - use evaluations to optimize safe and effective medication use for patients requiring palliative care

· Establish and maintain a collaborative practice agreement with the managing medical provide r

· Assist with the development and/or implementation of treatment guidelines, protocols and order sets

· Participate in local and system committees including but not limited to pain and palliative care.

· Serve as department of pharmacy services representative for all things pain and palliative care related during accreditation site visits

Development of postgraduate training opportunities (e.g., PGY2 pain and palliative care residencies)

EDUCATION/EXPERIENCE REQUIRED: Must hold a degree from an accredited college of pharmacy and must complete a residency training program or have equivalent experience in hospital pharmacy practice. Minimum of 3-5 years of pharmacist experience or the equivalent training and experience at another institution, preferred. Must have demonstrated the ability to meet and exceed practice standards set for a Level II pharmacist. (Henry Ford Detroit Hospital Only). Must be able to serve as an institutional authority in a given specialty practice area. Must have demonstrable leadership abilities and exhibit teamwork when working with others. Must have excellent verbal and written communication skills which enable him/her to contribute effectively in the dissemination of information to all levels of health care personnel in the institution. Should have a proven research/publication record. Must be qualified to work with pharmacy students at the undergraduate and graduate levels. Must have a previous work history of reliability, dependability, and willingness to assume additional responsibilities.

CERTIFICATIONS/LICENSURES REQUIRED: Must be licensed or eligible for licensure in the State of Michigan. If eligible, licensure must be obtained within 30 days of hire; failure to obtain licensure will result in termination of the Pharmacy Specialist position.
