Henry Ford Health System is Hiring a Chaplain (Palliative Care) - Fulltime - Days - Detroit Near Detroit, MI

GENERAL SUMMARY:
Provide a collaborative and inclusive approach to pastoral care by ministering holistically to the spiritual needs of patients, their families, medical staff and administration. Reports to the Supervisor of Spiritual Support Services.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
  • Attend daily interdisciplinary team meetings and collaborate with the interdisciplinary palliative medicine team, sharing important psycho-social, spiritual, and family dynamic concerns.
  • Participate in family care conferences with interdisciplinary team.
  • Help interpret spiritual distress and its effect on patient decision-making.
  • Provide spiritual and emotional support to patients of diverse backgrounds.
  • Develop relationships with community faith leaders and cultural brokers, building the sources of support available for specific needs
  • Provide institutional leadership through spiritual care education for all disciplines
  • Support end-of-life decision making and provide spiritual support during the dying process.
  • Perform Advanced Care Planning conversations and document in the EMR
  • Provide in the moment bereavement support
  • Collaborates with palliative social workers and nurses to perform bereavement follow-up calls and connect families with hospice and other community resources for follow-up bereavement support.
  • Develop self-care activities to engage the palliative medicine team and other hospital staff in self-reflection regarding their own spirituality.
  • Lead debriefing sessions for the palliative medicine team and other hospital staff after a difficult patient death.
  • Through structured activities and embedded in daily work, provides spiritual care education for the palliative care fellows, rotating learners, and team.

EDUCATION/EXPERIENCE REQUIRED:
•Master's degree in theology, Pastoral Studies, or a related field.
•Palliative Care and Hospice Advanced Certification (PCHAC) (with either NACC or BCCI or any accredited National certifying bodies), preferred or required within the first year of hire.
•For Chaplain only: Two (2) basic and two (2) advanced units of CPE toward certification within one year of hire. (Not required for Priest) Certification preferred.
•Minimum of 2-5 years' experience in spiritual care ministry in a hospital and or hospice care setting preferred.
•Endorsement from congregation leadership
